The Site ID sign and E/S Plan were present at the entrance of the access road. E/S controls appeared to be in place and working properly. The Company Man was not on site during inspection. A service company was on location drilling out plugs, and running 2-7/8"coil tubing to TD to clean and flush out hole. Once the hole is clean, the tubing will be pulled out, and flowback will resume. A two-stage frac was recently completed on the well. Stone indicated that flaring of the well will resume on 12/28/09.

1896134 Primary Facility 2009-12-16 Complaint Inspection Violation(s) Noted CW-complaint investigation ultimately determined Stang 1 Well had impacted quantity and quality of private drinking water well within 1000 feet of gas well, CO&A negotiated for Stone Energy to replace or restore potable drinking water to Clinton Property.
2005258 Primary Facility 2011-09-20 Follow-up Inspection No Violations Noted Follow up inspection of well due to recent flooding. Well is shut in. Pressure gauge on production = 0 psi. Visual evidence that pas was under water during flood. Appears to be no damange or impact on well. No one on site at time of inspection.
2005381 Primary Facility 2011-09-14 Routine/Complete Inspection No Violations Noted
2047822 Primary Facility 2012-03-05 Routine/Complete Inspection Pending well appears abandoned need to check status on situation.
2440857 Primary Facility 2016-01-14 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:50 a.m., the cellar was full of gravel, the vent pipe was set up with a 0-300psi pressure gauge that pegged the gauge, there was no gas detected at the perimeter of the cellar, there was about 1675psi on the production casing and about 1950psi on the tree top.

The well was granted inactive status on 3/26/2012. I called Afton Sterling of SWN about the gauge on the vent pipe, she said she would have someone meet me at the well and change the gauge. I left the site at 11:20 a.m.

2441072 Primary Facility 2016-01-15 Follow-up Inspection No Violations Noted At time of inspection, arrived on site at 10:15 a.m., I met with SWN field operator Ben Downey to check the pressure on the 8 5/8 vent pipe that had the 0-300psi pressure gauge that was pegged. Ben used a Taylor plug with a crystal pressure gauge, the pressure was 431psi.

I left the site at 10:30 a.m.

2591184 Primary Facility 2017-05-05 Routine/Complete Inspection No Violations Noted Routine (Pre-Plugging) Inspection of SWN’s Stang 1 (115-20156) gas well in Rush Twp., Susquehanna County conducted May 5, 2017 at 10 am. I met with SWN to check well pressures with a crystal gauge. The well is shut-in with 1608 PSI on the (4 ½”) production casing, and 726 PSI on the backside (4 ½” x 8 5/8”). 11 3/4” surface casing was buried under gravel in the cellar. No gas detected at ground level. I spoke briefly with the operator about the tentative plans for this well. In a couple of weeks they’ll set a bridge plug above top perfs and squeeze the backside to temporarily abandon prior to frack on nearby wells, during which they’ll monitor pressure on this well and reenter thereafter to complete the plug and abandonment.
2595868 Primary Facility 2017-05-18 Routine/Complete Inspection No Violations Noted Routine Inspection of SWN’s Stang 1 (115-20156) gas well in Rush Twp., Susquehanna County conducted May 16, 2017 at approximately 2:00 pm. Location was vacant at time of inspection. Temporary Abandonment operations have begun on the pad. Slickline was rigged up Monday, pumped brine through tubing to kill well and bledoff backside of production casing. The operator is waiting on rig to complete the Temporary Abandonment (rig broke down in route). Well has a production tree in place and is shut-in and isolated from pressure gauges. No obvious issues were observed at the time of this inspection.
2598074 Primary Facility 2017-05-25 Routine/Complete Inspection No Violations Noted Route inspection of the Stang 1 well conducted May 25, 2017 at 1:54 pm. The well had a production tree, 0 PSI on production casing (bridge plug set and cement plug above top perfs), and ~650 PSI on the backside according to analog pressure gauge. The operator intends to complete the P&A of this well after completion of nearby frack job. No violations were observed.
2620619 Primary Facility 2017-07-28 Routine/Complete Inspection No Violations Noted Route inspection of the Stang 1 well conducted July 28, 2017 at 10:30 am. The well is shut-in on production casing. No pressure on production casing gauge. Analog gauge for backside of production casing reads ~700 PSI.
2622475 Primary Facility 2017-08-03 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of the Stang 1 well conducted August 3, 2017 at 11 am. Rig on location for P&A. CIBP previously set at 3512’. At time of inspection; dropped ball activate gun perforated at 3508’. Attempted injection rate at 1200 PSI, held for 5 mins, no bleed off. Rig pulling out of hole, plan to run back in and shoot perfs at 3298’ and 3420’, planning to set retainer between sets and try to circulate for squeeze.
2625619 Primary Facility 2017-08-10 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of the Stang 1 well conducted August 10, 2017 at approximately 1 pm. Rig on location for P&A. At the time of inspection, rig tripping out with mill after milling ~11’ section in production casing after several failed attempts to inject behind casing. The operator intends to set cement plug across window to stop flow of gas on the backside of production casing.
2627867 Primary Facility 2017-08-16 Routine/Complete Inspection No Violations Noted Inspection of the Stang 1 well conducted August 16, 2017 at approximately 1 pm. Crew is off location following plugging operations, well is shut-in with master valve. No gauge for production casing. Analog gauge for backside of casing read 0 PSI, likely isolated at needle valve. Plugging not finished.
2638895 Primary Facility 2017-09-22 Plugging(Includes Plugged/Mined Through) No Violations Noted Well is SI on master valve. Backside is open and venting to gas buster. No gas detected at gas buster, checked w/Altair 5x gas meter. Operator has ceased P&A operations and monitoring well to evaluate effectiveness of remedial work.
2654934 Primary Facility 2017-10-27 Plugging(Includes Plugged/Mined Through) No Violations Noted Rig on location to finish P&A. Rig is in process of milling production csg near surface csg shoe. They'll mill ~20' of casing and pump resin across window to isolate any residual gas behind the production csg downhole. No obvious issues observed during the inspection
2658045 Primary Facility 2017-11-02 Plugging(Includes Plugged/Mined Through) No Violations Noted Rig is off location following plugging ops. Tree on well, operator left needle valves open prior to my arrival for wellbore access. No gas was detected at the valve for production csg, or at the valve for backside of production csg. No gas detected outside of 8 5/8" casing. I attached a balloon on each valve which the operator has left and monitored for 15 minutes. Neither balloon inflated. No obvious issues were observed.

There was standing water on the pad, of which no bubbling was observed. I checked several areas above the wells buried location for gas with an Altair 5x gas meter. No gas was detected. There were no obvious concerns observed. Attached is a photo from the inspection and a photo of the monument cap before burial.
2708131 Primary Facility 2018-03-15 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of the Stang 1 plugged unconventional well was conducted March 16, 2018.
