At the time of inspection, Frac Tech was on location with their fraccing equipment. Fraccing operations were temporarily shut down. According to Mr. Connolly, they were in the process of setting up to complete a 3-stage frac on the well. The TD of the well is reported as 6698'. Secondary containment was in place under trucks and equipment on site. Mr. Connolly identified the area of a recently reported frac fluid spill which was recently addressed and remediated.

2177440 Primary Facility 2013-06-12 Routine/Complete Inspection Violation(s) Noted Inspection conducted on 6/12/13 of the Stone Energy Loomis well pads located in Rush Township, Susquehanna County. The lower pad has drilled on it the Loomis 4H and the Loomis 1. The 4H has been plugged and has a metal plaque with the permit number on it on a metal pole presumably located directly above the well. The Loomis 1 is in production and has its associated production equipment located on the upper pad. Black staining is located on this pad near the freshwater impoundment. The staining has a distinctive petroleum odor and will need to be remediated ASAP. Erosion is occurring on the pad fill slopes. No E&S plans on site. A set of plans need to be on site. No E&S inspection records on site. At least 8 weeks’ worth of E&S inspections needs to be kept on site. Please provide copies of your last 6 months of E&S inspections conducted for this site.

2314745 Primary Facility 2014-10-10 Incident- Response to Accident or Event Pending Arrived at the site at 14:45. Afton Sterling, Regulatory Analyst Supervisor of Southwestern Energy Production Company contacted the Department on 10/10/14 to report a spill on the Loomis Lower Pad. She reported that a well tender has been dumping production fluid on the Loomis Lower Pad for quite some time, and the appearance of the spill suggests that is extensive. The lower pad has one vertical well, but does not contain any production equipment. Production fluid flows via an underground pipeline to the production fluid storage tanks on the Loomis upper pad. The Production fluid tanks contain fluid from the well on the lower pad and the (2) wells on the upper pad. The Loomis property is in the process of ownership transformation from Stone Energy to SWN.

Southwestern Energy personnel at the location during the inspection were Mike Deming, Regulatory Analyst, Justin Phillips, Health Safety and Environment Staff co-coordinator and Dave Bohman, Production Foreman. Leidos Engineering was at the site in preparation to collect split samples with the Department along with perform soil conductivity screening of the area of the lower Loomis Pad impacted from the production fluid release. The process of screening the soil for conductivity will aid in determining the extent of the excavation and evaluating conditions of the surface of the pad impacted from the release.

2506025 Primary Facility 2016-07-29 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:15p.m., the cellar was about half full of gravel with no gas detected at the perimeter of the cellar, the (8x4) vent pipe and the (4/prod) are both set up with a Tayler plug to check pressure with a Crystal pressure gauge. There is a production line going from the well and it appears that all of the valves are closed with a chain connected to all of the valves with a lock, all the pressure gauges on the well are showing 0psi and there are no tanks on the site. I took a walk around the location and everything looked OK.

There were no obvious concerns noticed. The well was put on inactive status on 12/7/15. I left the site at 12:35p.m.
