The Site ID sign and E/S Plan were present at the entrance of the access road. At the time of inspection, there was no drill rig on site. The contractor (Barber and Deline) were on site loading materials to be moved off location. Barber and Deline were contracted to set the conductor and mouse hole, which were observed on location. Metal caps were welded on each.
1825891 Primary Facility 2009-08-27 Drilling/Alteration No Violations Noted An existing road was used to access the well site. The road and drill pad are stoned with limestone. The Site ID sign and E/S Plan were present at the entrance of the access road. At the time of inspection, drill rig Pioneer #60 was on site. I met with Company Man John Mode who informed me that the current depth of drilling was 1226'. Mr Mode indicated that they were currently drilling down to a projected depth of 1445' at which point they will be setting 9-5/8" surface casing. Cementing of the surface casing would likely occur sometime this evening according to Mr. Mode.

The site was in good shape, and had no issues of any consequence. The site work followed the plans.

There were no erosional issues with the site, all slopes were stable and there were no failures of BMPs to note.

2505614 Primary Facility 2016-07-26 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:25a.m., the cellar was full of gravel and no gas was detected. The vent pipe was open and set up with a Taylor plug; there was 199psi on the production casing and 112psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 10:50a.m.

2599629 Primary Facility 2017-05-31 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 8:30a.m.

The cellar was full of gravel with water setting on top with bubbling noticed. I detected gas at 100% LEL and peaked at 26% methane. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge. There was 395psi on the production casing and 109psi on the production line. I took a walk around the location and there were no other obvious concerns noticed. I left the site at 9:00a.m. I contacted Steve Moyer of SWN to set up a follow up inspection to determine where the bubbling is coming from.

2602042 Primary Facility 2017-06-06 Follow-up Inspection No Violations Noted At time of inspection, arrived on site at 8:50a.m. for a follow up inspection.

During a routine inspection conducted on 5/31/17, bubbling was noticed in the cellar and gas was detected. I met with Cody Stanton (production foreman for SWN). The cellar was full of water and a water truck was on site to vac the water level down so the source of the bubbling could be determined. The bubbling was coming from the treads of a bull plug that was screwed into the production head across from the (9x5) annulus vent pipe. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, Cody check the pressure at 117psi. I left the site at 9:20a.m.

2704687 Primary Facility 2018-03-13 Routine/Complete Inspection No Violations Noted At time of inspection.

The cellar was set up with a corrugated pipe around the well for inspection access with water 1' below grade. The (20x13) and (13x9) annuli were below the water level and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water 1' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
