I took a walk around the location, everything looked OK.
2484448 Primary Facility 2016-05-03 Routine/Partial Inspection No Violations Noted The Department was on site at the Ivey Well Site to conduct a Routine Partial Inspection. At the time of the inspection, the Department noted that the E&S Inspection Checklist noted that an inspection was conducted on 4/28/16. The Department noticed that there was a redline change on the plan map from 10/18/11 that stated that Carrizo’s access road is in a forested wetland and that they were notified. Also noted on the plan map was that the UNT to Fall Brook was unclassified under Chapter 93. The Department requests that the operator investigate the stream classification of the UNT to Fall Brook. Between the forest and the access drive is an area that appears to have been re-vegetated. Tree tubes and wetland vegetation were noted in the area.

2513782 Primary Facility 2016-08-25 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:25p.m., the cellar was full of gravel and no gas was detected. The vent pipe was open and set up with a Taylor plug to check for pressure, there was 257psi on the production casing and 240psi on the production line. I took a walk around the location and everything looked OK.

2528853 Primary Facility 2016-09-29 Routine/Complete Inspection No Violations Noted The Department, represented by Frank Hoehle and Jon Sassi were on site to conduct a RTNC inspection of the Ivey GU1H well site. The ESX number for this site is ESX11-115-0019.

There was a large spoil pile on the North Northwest corner of the pad, and according to Afton Sterling, SWN representative, it was inherited when SWN took control of Alta holdings. The material appears to be rock and there was no sheen or high conductivity in the area. The well has its own compressor station and has a plunger lift installed to help boost production. There is no containment under GPU.

There were no erosional issues with the site, all slopes were stable and there were no failures of BMPs to note.

2608740 Primary Facility 2017-06-28 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:15a.m.

The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge. There was 471psi on the production casing and there was no reading on the Schneider digital pressure gauge on the production line. The well was not producing at time of inspection, the valve at the GPU was closed. I took a walk around the location and there were no obvious concerns noticed. I left the site at 11:45a.m.
