2423968 Primary Facility 2015-11-10 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:00 a.m., the cellar was full of gravel, there were two vent pipes with no tags venting into the atmosphere, there was no gas detected from either vent pipe, there was gas detected at the perimeter of the cellar at 18% combustible, there was 357psi on the production casing and 267psi on the production line. I met with well tender Tyler Rowe to determine the source of the gas. Tyler dug a hole about one foot deep where gas was detected, I checked for gas at the flange of the well head and there was no gas detected. I noticed a drip from a fitting on the well head, Tyler tightened the fitting and it was determined that it was the source of the reading. I took a walk around the location and everything looked OK.

The vent pipe was open and set up with a Taylor plug to check for pressure, there was 217psi on the production casing and 111psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 13:10p.m.

2538282 Primary Facility 2016-11-21 Site Restoration No Violations Noted The Department (Steve Brokenshire and I) was on site to conduct a Restoration Inspection in response to the submittal of a Notice of Termination (NOT).

The E&S Inspection Checklist indicated that on 11/14/2016, S. Maya performed a weekly inspection and noted that no attention was needed.

There appears to be landowner disturbance on the NW side of the well site. Palletized stone and 5 gallon buckets are situated in this area.

The cut slope and fill slope are comprised of a mixture of vegetation and stone.

Areas adjacent to the access road do not appear to meet permanent stabilization requirements. Included with the NOT was an email explaining that the landowner would seed and mulch this area but regardless, it does not meet permanent stabilization requirements, therefore I recommend that this NOT be denied.

2613971 Primary Facility 2017-07-10 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:55p.m.

The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge. There was 205psi on the production casing and 166psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:25p.m.

The well was in production. The cellar was full of gravel and dirt, I was unable to check for gas at the perimeter of the cellar because the ground was frozen. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

The well was shut in at the GPU. The cellar was full of gravel and was unable to check for gas at the perimeter of the cellar because the gravel was frozen. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
