The (9x5) vent pipe was open and connected to the production line, there was 333psi on the production casing and 295psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 13:45p.m.

2593766 Primary Facility 2017-05-10 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 14:35p.m.

The cellar was full of gravel with water setting on top with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge and connected to the production line, the valve going to the production line was closed. There was 311psi on the production casing and 273psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 15:10p.m.

2607821 Primary Facility 2017-06-27 Routine/Complete Inspection No Violations Noted The pad surface was solid and had grasses germinating through the stone on the pad surface. The cut and fill slopes were solid and well vegetated. The last company inspection was a post-storm inspection on 6-6-2017. The well cellar was filled with crushed stone so no bubbling could be determined, and the production tank containment was about one third full of water, possibly due to recent rains. There was a spoil pile on site but no dirty runoff was observed from the pile. The super silt fence behind the compressor station on site is in need of removal or repair. There is adequate vegetation throughout the area.

There were no violations noted.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

The well was in production. The cellar was full of gravel with water on top and there was bubbling noticed. I contacted Steve Moyer of SWN to let him know of the bubbling. I suggested the gravel be removed from the cellar and contact me when it is completed to set up a follow up inspection to determine where the bubbling was coming from. I took a walk around the location and there were no obvious concerns noticed.

2801925 Primary Facility 2018-11-13 Follow-up Inspection No Violations Noted At time of inspection.

The well was in production. On site for a fallow up inspection for bubbling noticed in the cellar during a routine inspection conducted on 11/6/18. The gravel has be removed from the cellar as suggested to determine where the bubbling was coming from. I met with Ryan Klemish, Wyatt Tompkins and Shane Beamer of SWN. Wyatt used a flare camera to check for gas presence in the cellar. After noticing gas from the side of the well, Shane then sprayed windex on the well head (see photo #4). After further inspection of the well head, it was determined the gas was leaking from a 1" recess plug on the (9x5) section on the well head (see photo #5). Shane checked the pressure on the (9x5) with a Crystal pressure gauge, he got a pressure reading of 585psi. Ryan said they will bleed the pressure off the (9x5) annulus and have the plug tighten or replaced. There were no violations noted.
