Company man arranging for vac truck to be on site in morning to pump down cellar in order to investigate bubbling. DEP will be on site tomorrow.

2110148 Primary Facility 2012-10-11 Follow-up Inspection Violation(s) Noted On 10/10/12 DEP received a call from WPX Energy reporting bubbling in the cellar of this well. On 10/11/12 DEP arrived on site to inspect the well and reported bubbling condition. At the time of arrival the cellar was full of fluid. As requested, WPX arranged for a vac truck to be on site to pump out the standing fluid while DEP was on site. After pumping the fluid from the cellar, the source of the bubbling was determined to be coming from the 20" x 13 3/8" annulus. Cement is present at the top of the 13 3/8" casing. Consistent bubbling is present coming through the cement in this annulus. While on site, I informed WPX that an NOV will be issued for the current conditions of the well. In the meantime, I requested the Operator prepare to install a collar and vent line with gauge to allow collection of gas samples and to monitor pressure.

2433863 Primary Facility 2015-12-15 Routine/Complete Inspection Violation(s) Noted At time of inspection, arrived on site at 11:45 a.m., the cellar had water at the bottom, a collar was on the (20x13) annulus with a vent pipe that was open and venting into the atmosphere and gas was detected at 100% LEL and 100% methane with an Altair 5X multi gas detector which was calibrated 12/15/15, the (9x5) vent pipe was open and set up with a Taylor plug, there was 371psi on the production casing and 369psi on the production line. I took a walk around the location and everything looked OK.

2556428 Primary Facility 2017-01-27 Incident- Response to Accident or Event Violation(s) Noted I traveled to the WR 73- Webster Well Site in response to a complaint of mud being tracked from the SWN Webster access road onto Hawk Road. At the time of my arrival, Jon Sass (DEP-EPS) and I walked along Hawk Road and found that sediment was leaving the well site and flowing down the roadside ditch heading east in said roadside ditch along Hawk Road. Mr. Sassi found that a cross culvert on Hawk Road was carrying the sediment from the north side of the road out to the southern side of the road and depositing it in a stream channel. It appears that the stream channel is the headwaters of DuBois Creek (CWF, MF). The roadside ditch was nearly full of sediment east of the well site. Any water moving through the ditch in this area was heavily sediment laden. I found that the roadside ditch west of the well site was running clear and nearly sediment free aside from the 2’-3’ from the access road where it appeared that some sediment found its way into the ditch.

2561724 Primary Facility 2017-01-31 Follow-up Inspection Outstanding Violations - Viols Req'd The Department was on site to meet with Seth Tratthen and Steve Moyer (SWN) to follow up on the Department’s Notice of Violation dated 1/27/2017. At the time of my arrival, I walked along the access road. The site was snow covered and it was currently snowing during the inspection. I moved some small areas of snow aside to view the status of the access road after SWN reported to have repaired it. I noted that the stone on the surface of the access road does not appear to be the proper size as denoted in the Erosion and Sediment Pollution Control Program Manual.

Mr. Moyer explained that they were advised to “choke” the access road by placing smaller sized material in an effort to solidify the road. We then walked to the base of the access road and viewed the Hawk Road Roadside ditch, which although snow covered, was found to have been reworked and the stone that they used was visible through the snow cover. 2561920 Primary Facility 2017-02-08 Incident- Response to Accident or Event Violation(s) & Outstanding Violations The Department (Jon Sassi-DEP, EPS) received a phone call from Mike Demming (Senior Regulator Analyst-SWN) on 2/7/17 at 1601 hours to self report a release of less than 5 gallons of diesel fuel. The release was discovered at 1405 hours. Mr. Demming explained that a US Well Services heater trailer (line heater which keeps water from freezing during hydraulic fracturing operations and has since been taken to their yard for repair) was exiting the location and was tilting forward on the access road when the diesel fuel overflowed the belly pan and was released to the access road. The diesel fuel traveled along the access road being carried by stormwater (raining at the time of the release) downslope and flowing into the roadside ditch along Hawk Road. The belly pan is used to collect leaks and overflow from the heater trailer. The operator failed to clean the belly pan prior to demobilization. A mini excavator will be used to remove any impacted material and a vac truck will remove and impacted fluid. S
2572189 Primary Facility 2017-03-09 Follow-up Inspection No Violations Noted We met with Mike Deming (Senior Regulatory Analyst-SWN) and Adam Brace (Production Foreman-SWN) during this inspection. During the inspection, the Department was approached by a complainant who voiced their issues concerning the condition of Hawk Road. They explained that the increased truck traffic due to activity at the well site has pushed material into the roadside ditch (because the trucks are driving at the edge of the roadway)causing sediment laden water to enter the stream.

They requested that the Department have SWN repair Hawk Road and install catchment basins at the culvert outlets along the roadway. The complainant explained that when Alta was operating the site, that a similar complaint was made due to sediment entering a pond in the vicinity. They showed us an area where they said rock was added to the roadside ditch in response to that issue.

2623871 Primary Facility 2017-08-09 Complaint Inspection No Violations Noted I met Mike Deming (SWN) on site in response to a complaint of "green sludge" in the Hawk Rd. roadside ditch. At the time of my arrival, it was noted that the roadside ditch was green. Upon inspection, it was found that a dense mat of what appeared to be filamentous algae was growing. In addition to the algae, I discovered some areas with iron/iron bacteria seeps entering the ditch. There were also multiple frogs inhabiting this area. I rearranged some of the algae, as to not disturb the frogs, so that I was able to check the clarity of the water. The water appeared to be clear, no sheen was noted. At this time I recommend that the complaint be closed as the source was natural.
2668900 Primary Facility 2017-12-08 Routine/Complete Inspection No Violations Noted I performed a routine complete inspection of the Webster well pad on December 8, 2017. The pad was stable, as well as the access road. The cut and fill slopes had vegetation where it was able to grow, but it was fairly sparse due to the very rocky nature of the soil. The wells onsite were labeled. The containment on site was not full and there did not appear to be any erosional problems at the time of inspection.

There were no violations noted.

2695211 Primary Facility 2018-02-15 Routine/Complete Inspection Outstanding Violations - Viols Req'd At time of inspection.

The well was in production. The (16x13) annulus was venting to the atmosphere and gas was detected. The (13x9) annulus was below the water level and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed.

2794459 Primary Facility 2018-10-25 Routine/Complete Inspection Outstanding Violations - Viols Req'd At time of inspection.

The well was in production. There was water 2' below grade in the cellar and there was no bubbling noticed. There was a role off container on containment (see photo) that had dirt in it that was scraped up with foaming agent, the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
