Have not gauged since squeeze. While on site, asked Co. Man to update site id sign.

Spilled 135 bbls drilling mud onto plastic. Spill occurred at 18:30 on the 3rd. Hands and vac trucks used to clean up spill. Violation cited of general requirements - failure to control fluids. Appears that mud was contained on plastic. I informed Operator that DEP will do follow up inspection when plastic is removed from site when drilling complete. It was strongly suggested to Operator to use factory hoses from solids control company on discharge side of pumps.

I took a walk around the location and everything looked OK.

I left the site at 12:25 a.m.

2517054 Primary Facility 2016-08-31 Site Restoration No Violations Noted The Department was on site to conduct a Notice of Termination (NOT) for the Joseph Blaisure Pad 1 well site (Erosion and sediment permit 58-09-802). The wells associated with this site are as follows:

The site was well vegetated with the correct perennial grasses, over the 70 percent perennial vegetative threshold and the earthwork was performed to specifications. There were no erosion or sedimentation issues with this site.

2548707 Primary Facility 2016-12-22 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:30p.m., the cellar was full of ice. The (13x9) vent pipe was open and venting into the production tank, the (9x7) vent pipe was open and venting into the production tank, the (7x4½) vent pipe was open and venting into the production tank. There was 576psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 13:15p.m.

2636144 Primary Facility 2017-09-14 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. All the annuli vent pipes were connected together and venting into the same tank. I took a walk around the location and there were no obvious concerns noticed.

2728173 Primary Facility 2018-05-08 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection for the Jo Blaisure pad. The wells associated with this pad are as follows: 115-20239, 115-20245, 115-20399, 115-20400, 115-20401. The wells were all properly labeled.

The access road and pad surface were solid and stable with no erosional issues.

The cut and fill slopes were stable and well vegetated and were greening up nicely. There did not appear to be any erosional issues.

There were no violations noted.

2743794 Primary Facility 2018-06-20 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 1.5' below grade in the cellar and there was no bubbling noticed. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank at 100% LEL and 100% methane. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2838580 Primary Facility 2019-02-11 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of ice and I was unable to check for gas. The (13x9), (9x7) and (7x4) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 2/5/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
