On September 22, 2011, I approved of an alternative waste management practice (OG-71) for Williams to conduct filtration of frac flowback fluid at the Depue Well Pad via an on-site mobile filtering unit with a pump and allow for subsequent reuse of the treated water at the Knosky Pad. The filtration process will separate solids and metals from the fluids. Solids will be hauled off-site for landfill disposal. This entire operation is located on secondary containment. I observed a petroleum like sheen in puddled water on containment in a close proximity to the residual waste vacuum truck involved in the filtration process. There was a hole in the containment liner nearby the sheen. The puddled water containing the sheen was not impacting the area in the liner were the tear was observed. |

There was 364psi on the production casing and 369psi on the production line. There were three other wells on site, the (115-20459 4H), (115-20461 8H), and (115-20462 10H) that were in production. There were no obvious concerns noticed at time of inspection. I left the site at 13:10p.m. |
