DEP were informed by WPX that they would begin immediately addressing the E&S issues, failures of containment and potential pollution that was noted.

DEP observed contractors conducting repairs to the site and discussed some possible upgrades or solutions to some of the issues. Outstanding violations still exist, but no violations were noted during our inspection. Please contact DEP with any questions. Subsequent inspections conducted at site found conditions to be suitable.
1974908 Primary Facility 2011-05-11 Routine/Complete Inspection Pending At time of inspection crews on site cleaning up pad. removing plastic and scraping off any residual waste from pad surface. observed bubbling in between 9 x 13" annulus and 13 x 16" annulus. Further investigation will be completed by DEP. Rathole has not been filled in, however, Wiliams rep. informed me all ratholes were going to filled in tomorrow or next day. DEP will perform FUI.
2026347 Primary Facility 2011-12-08 Routine/Complete Inspection No Violations Noted I arrived at the site at 13:10. On September 22, 2011, I approved of an alternative waste management practice (OG-71) for Williams to conduct filtration of frac flowback fluid at the Depue Well Pad via an on-site mobile filtering unit with a pump and allow for subsequent reuse of the treated water at the Knosky Pad. The filtration process will separate solids and metals from the fluids. Solids will be hauled off-site for landfill disposal. This entire operation is located on secondary containment. I observed a petroleum like sheen in puddled water on containment in a close proximity to the residual waste vacuum truck involved in the filtration process. There was a hole in the containment liner nearby the sheen. The puddled water containing the sheen was not impacting the area in the liner were the tear was observed.
2027744 Primary Facility 2011-12-12 Follow-up Inspection No Violations Noted I arrived at the site at 13:00. R&T Oilfield Services Inc. was on-site and in the process of filtering frac flowback fluid related to the OG-71 I approved of. I spoke with Jason Ridgeway and Zach Cook of R&T regarding the filtering operation. Gas flaring at the site still on-going as observed in the 12-8-11 inspection.

2434451 Primary Facility 2015-12-21 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:15 a.m., a crew was on site plugging the 6H, the cellar was full of gravel, the vent pipe was open and set up with a Taylor plug, there was no gas detected at the perimeter of the cellar, there was 315psi on the production casing and the valves were closed on the production line while the 6H was being plugged. I took a walk around the location and everything looked OK.
2530418 Primary Facility 2016-10-20 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:45p.m., the cellar was full of gravel with water setting on top with no bubbling noticed. The vent pipe was open and set up with a Taylor plug to check for pressure. There was 366psi on the production casing and the valve was closed on the production line. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 13:35p.m.

2538849 Primary Facility 2016-11-21 Routine/Complete Inspection No Violations Noted Inspection for inactive status request received on November 17, 2016.

At time of inspection, arrived on site at 12:40a.m., the cellar was full of gravel and water with no bubbling noticed, the vent pipe was open and set up with a Tayler plug to check for pressure. I met with Tyler Rowe (well tender for SWN), he used a Crystal pressure gauge to check the pressure on the vent pipe, and the gauge reading was -1.5psi. There was 364psi on the production casing and 369psi on the production line. There were three other wells on site, the (115-20459 4H), (115-20461 8H), and (115-20462 10H) that were in production. There were no obvious concerns noticed at time of inspection. I left the site at 13:10p.m.

2619630 Primary Facility 2017-07-26 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:40p.m.

The cellar was full of gravel with water setting on top with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge. There was 356psi on the production casing and 355psi on the production line. The main valve on the well and at the GPU were closed. The well was granted Regulatory Inactive Status on 2/2/17. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:35p.m.

2727567 Primary Facility 2018-05-08 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was granted Regulatory Inactive Status on 2/2/17. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The main valves on the production line and at GPU were closed. There were two 2100 gallon tanks labeled 70% Non Potable Water and 30% methanol on containment and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

The well was shut in and was granted regulatory inactive status on 2/2/17. The cellar was full of gravel and I was unable to check for gas at the perimeter of the cellar do too the gravel being frozen. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
