2434448 Primary Facility 2015-12-21 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:15 a.m., a crew was on site plugging the 6H, the cellar was full of gravel, the vent pipe was open and set up with a Taylor plug, there was no gas detected at the perimeter of the cellar, there was 333psi on the production casing and the valves were closed on the production line while the 6H was being plugged. I took a walk around the location and everything looked OK.
2530420 Primary Facility 2016-10-20 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:45p.m., the cellar was full of gravel with water setting on top with no bubbling noticed. The vent pipe was open and set up with a Taylor plug to check for pressure. There was 235psi on the production casing and 178psi on the production line. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 13:35p.m.

2619638 Primary Facility 2017-07-26 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:40p.m.

The cellar was full of gravel with water setting on top with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge. There was 224psi on the production casing and 165psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:35p.m.

2727568 Primary Facility 2018-05-08 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. There were two 2100 gallon tanks labeled 70% Non Potable Water and 30% methanol on containment and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2826601 Primary Facility 2019-01-14 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was shut in at the GPU by a pneumatic valve. The cellar was full of gravel and I was unable to check for gas at the perimeter of the cellar do too the gravel being frozen. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
