At the time of inspection crews were on site installing new plastic secondary containment for upcoming frac. Six wells have been drilled on location (1H - 6H). Al six wells are currently shut in and not vented. Inspection of Baker 1h revealed minor bubbling around well head. Due to the level of fluid in the cellar, the source could not be determined. It is requested by the DEP that the source of the leak be identified. The Operator is to notify the DEP when this work is to be completed. The Operator shall provide a sumary of the findings, and a description of what reparis need to be completed. According to Chapter 78 Section 78.86 Defective casing and cement...the Operator has the option to correct the defect or submit a plan to correct the defect for approval by the DEP within 30 days. |

Conductivity readings in the sediment trap indicated a high reading of 2.18 milliSiemens/cm. Salinity indicated 1.5 parts per thousand and Total Dissolved Solids indicated 1.5 parts per million and 1.5 grams per liter. Conductivity, Salinity and TDS readings indicated over the limit, with the limit on the meter being 19.99 milliSiemens/cm, in the duck pond secondary containment where the pump was stored as part of the filtering operations. I also observed a petroleum sheen inside this containment area as well. A small area of ponded water located just outside this same area indicated Conductivity, Salinity and TDS readings of over the limit. The ponded water was on the Well Pad off secondary containment. |

The area was stable and had no erosional issues at this time and it is recommended that this Notice of Termination be APPROVED. There were no violations. 2851751 | Primary Facility | 2019-03-11 | Routine/Complete Inspection | No Violations Noted | I performed a Routine Complete (RTNC) inspection of the Baker Well Pad on March 11, 2019. The weather was sunny and relatively warm, around 38 degrees.
The access road was a bit muddy but not unstable. The access road is quite long, passing some small logging operations, as well as a target shooting range situated along its length. The well pad surface was plowed and mostly wet but stable. The wellheads were encased in a carport structure. They were surrounded by clean gravel and were properly labeled. The containment surrounding the production tanks and apparatus was mostly empty and had sufficient freeboard. The cut and fill slopes of the site were rocky and stable. Where there was sufficient soil, there was adequate vegetation. No areas of instability were noted. The impoundment on site was full to capacity. It would be recommended to draw down the impoundment to prevent overflow and possible resulting E&S issues. There were no violations noted |
