2422220 Primary Facility 2015-11-04 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:20, the cellar was full of water, the (20x13) vent pipe was open and venting to a tank, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there were no pressure gauges on the well, I took a walk around the location and everything looked OK.

The site was in production, and not under any major construction at this time and the site was very well vegetated (>70%) and graded well. Surrounding the site active agriculture (haying operation) was occurring. There were no erosion and sedimentation issues on site and the access road was holding up well.

2516415 Primary Facility 2016-08-30 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:30a.m., the cellar was full of water with no bubbling noticed, the (20x13) vent pipe was open and venting into a tank, the (13x9) vent pipe was open and venting into a tank, the (9x5) vent pipe was open and venting into a tank. There was 471psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 10:15a.m.

2549893 Primary Facility 2017-01-05 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a routine complete inspection of the erosion and sedimentation controls at the Fraser E. well pad (ESX10-115-0020) on June 4, 2015. The wells associated with this pad are as follows:

The site was in production, and not under any major construction at this time and the site was very well vegetated (>70%) and graded well. The vegetation surrounding the site has been mowed, and is in great shape. There were no erosion and sedimentation issues on site and the access road was holding up well.

2568312 Primary Facility 2017-02-23 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete Inspection. This well site's ESCGP permit number is 5809802. I reviewed the E&S Inspection Log to find that Mike Brilla (TEC) conducted the most recent inspection on 2-14-17. He identified it as a post precipitation inspection.
2596338 Primary Facility 2017-05-17 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:20a.m.

There was a service crew on site to run 2¿ tubing in the wells to enhance production. I met with company man Jim Restly of Restly Oil & Gas Consulting. The tubing was pulled out of the 1V well and will be run in the 5H well. The 3H well has tubing in it and an additional 300' to 400' will be added. The 7H and 8H wells do not have tubing in them but will have new tubing run in them. The production units are also being refurbished. I took a walk around the location and there were no obvious concerns noticed. I left the site at 11:25a.m.

2703567 Primary Facility 2018-03-06 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was snow and ice 4' below grade in the cellar and was unable to check for gas. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2710121 Primary Facility 2018-03-26 Routine/Complete Inspection No Violations Noted I inspected the site on March 26, 2018. The weather was sunny and approximately 40 degrees. The access road was solid and stable. The pad surface was solid and the cut and fill slopes were well vegetated and stable. The wells were labeled clearly. The wells associated with this site were as follows:

2740434 Primary Facility 2018-06-12 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Fraser Pad. The wells associated with this are the 115-20447, 115-20463, 115-20464, 115-20473, 115-20474, 115-20477 (plugged), 115-20493 (plugged). The wells were properly labeled.

The access road was solid and stable as was the pad surface. There was no puddling onsite. There did not appear to be any issues of concern with the site stability.

The cut slope as well as the fill slope was stable and well vegetated. They were gently sloped and there did not appear to be any erosional issues.

There were no violations.

2800735 Primary Facility 2018-11-08 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water .5' below grade in the cellar and there was no bubbling noticed. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 11/2/18. There were five 55 gallon drums on containment (see photo) marked paint waste, a crew from Covanta Environmental Solutions arrived on site to remove the drums as I was about to leave. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
