Constant bubbling observed between 9 5/8 and 13 3/8" casing strings. Audible gas leakingout of vent on 9 x 5" was heard while on site. Water level in cellar was just below 13 3/8" casing top. Pipeline crews working nearby.
2098532 Primary Facility 2012-08-27 Follow-up Inspection No Violations Noted On site at 9 am. Followup on 72 hour shut in on 9 5/8 x 13 3/8" annulus. Installed 30 lb gauge on annulus on Friday 8/24/12 at 10 am. At time of inspection, gauge was pinned at 30 +lbs. At 9:40, ERM and WPX arrived on site. At 10 am, Kevin Costellor (DEP) on site. Locks were removed from valves and 30 lb gauge removed and replace with 2000 lb gauge. This gauge indicated a pressure build of slightly more than 20 lbs. DEP requested Operator get a smaller gauge (100 lb) to obtain a more accurate reading. At 11 am - 100 lb gauge on site. Pressure build with this gauge showed 34 lbs. ERM collected isotube and isotopic bag samples from needle valve on vent line. Due to the fact that 30 lb gauge was too small for initial SIT, DEP requested Operator re-do shut in with 100 lb gauge. TomorrowDEP will return to site with flow meter to obtain flow reading, and commence second 72 hr shut in. DEP offsite 12 pm.
2098553 Primary Facility 2012-08-28 Follow-up Inspection No Violations Noted On site 9 am. On site to take flow readings and start 72 hour shut in of 9 5/8 x 13 3/8" annulus. WPX on site. Kevin Costello (DEP) and Ryan Klemish (DEP) on site at 10:30. Used a Magna Helic flow meter to gauge flow. At 10:37 started flow using 0-8" water gauge. At 10:42 stopped and recalibrated gauge to zero. At 10:43 resumed flwo for two minutes - not seeing any flow. At 10:45 changed to different flow meter. Installed 0 - 50" water meter and resumed flow test from 10:46 to 10:56. Reading was +/- .1 inch which = about 52.8 cu/ft over 24 hours. with 1/8" orifice. At 10:58 shut in annulus for 72 hour shut in with 100 lb gauge. Will return to site on Friday Sept. 31 to check gauge and end shut in test. DEP offsite at 11:30.
2099644 Primary Facility 2012-08-31 Follow-up Inspection No Violations Noted On site at 10:30. WPX on site 10:30. ERM on site 10:45. At time of arrival - crew on site blowing down wells (fluid) to holding tanks - Wells 1,3,4. Swabbed 1H on Wednesday (Parker Energy Services). Crew finished blowing down wells @ 10:45 am and mobilized offsite. At 11:00 am - opened Hollenbech 2H 9 5/8" x 13 3/8" annulus. Reading on 100 lb gauge = 34 lbs after 72 hrs. ERM collected two (2) isotube samples. WPX opened vent and are keeping open at this time. DEP offsite at 11:30 am.

The vent pipe was open and set up with a Taylor plug to check for pressure, the valve on the vent line going to the tank was closed. There was 297psi on the production casing and 252psi on the production line. I took a walk around the location and there were no obvious concerns noticed.

2619620 Primary Facility 2017-07-25 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 14:55p.m.

The cellar was about half full of water with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 403psi on the production casing and 349psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 15:35p.m.

2726969 Primary Facility 2018-04-27 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 2' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2825819 Primary Facility 2019-01-11 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was shut in at the GPU by a pneumatic valve. There was ice 2' below grade in the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2845378 Primary Facility 2019-02-27 Routine/Complete Inspection No Violations Noted I performed a site inspection of the Hollenbeck well pad on February 27, 2019. The weather was light snow and temperature in the mid 20s.

The well pad was compact and fenced in along the roadside. There was no activity on site during the inspection. The access driveway and well pad surface was frozen hard.

The slopes surrounding the site were all very well covered with last years vegetation in abundance. There were no noted areas of slope failure or slumping.

All wells were properly labeled.

There were no erosional issues noted.

There were no violations noted at the time of inspections.
