The wells on this site are the 2H, 3H, 4H, 5H, 6H and 7H. The Numbers, respectively, are 115-20516, 20515, 20560, 20518, 20561, and 20463. The site's last E&S inspection was on 1-7-2015. The site was adequately vegetated and stable. There were no violations.
2377233 Primary Facility 2015-06-04 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a routine complete inspection of the erosion and sedimentation controls at the J. Hawley well pad (ESX10-115-0061) on June 4, 2015. The wells associated with this pad are as follows:

2505648 Primary Facility 2016-07-28 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 8:55a.m., the cellar was about half full of water with no bubbling noticed, the (13x9) vent pipe was open and venting into a tank, the (9x5) vent pipe was open and venting into a tank, there was 587psi on the production casing. There was a crew doing remedial work on the lower side of the location, I took a walk around the location and everything looked OK.

I left the site at 9:50a.m.

2549833 Primary Facility 2017-01-05 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a routine complete inspection of the erosion and sedimentation controls for the at the J. Hawley well pad (ESX10-115-0061) on June 4, 2015. The wells associated with this pad are as follows:

5H 115-20518 6H 115-20561 7H 115-20463 2H 115-20516 3H 115-20515 4H 115-20560

The site was in production, and not under any major construction. and the site was very well vegetated (>70%) and graded well. There were no erosion and sedimentation issues on site and the access road was holding up well.

2600900 Primary Facility 2017-06-01 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 14:05p.m.

The cellar was about three quarters full of water with no bubbling noticed. The (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. There was 562psi on the production casing and 469psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:45p.m.

2648774 Primary Facility 2017-10-18 Routine/Complete Inspection No Violations Noted I inspected the J. Hawley pad for Notice of Termination on October 3, 2017. The site was well constructed and had no apparent erosional problems. The vegetation was at or exceeding 70%.

There were no violations noted.

I recommend APPROVAL of this NOT.

2709554 Primary Facility 2018-03-16 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water and ice 1.5' below grade in the cellar and there was no bubbling noticed. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2710106 Primary Facility 2018-03-26 Routine/Complete Inspection No Violations Noted I inspected the J. Hawley well pad on March 26, 2018. It was sunny and about 38 degrees out. The access road was solid and the pad surface was partly snow covered yet stable. The cut and fill slopes were not steep and were well vegetated. The wells associated with the site are as follows:

There were no apparent erosional issues. There were no violations noted.

2808823 Primary Facility 2018-11-30 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of water and there was no bubbling noticed. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 11/2/18. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
