2084681 Primary Facility 2012-07-19 Incident- Response to Accident or Event Violation(s) Noted Arrived at the site at 12:35 AM in response to a flowback fluid spill that occurred at the site on 7-18-12 at 10PM. Scott Aharonian, EHS advisor of WPX reported the spill to the Department at 9:45 on 7-19-12. The estimated volume of the spill was determined to be (20) gallons. Mr. Aharonian informed me that the spill occurred during a hose disconnection as a vacuum truck was in preparation to transport the flowback fluid out of a frac tank to a permitted disposal facility. The flowback fluid spill impacted the surface of the ground on the Well Pad. The frac tank in relevance to the spill was one of the last tanks at the site being emptied. Stony Mt. Acres was on-site in the process of excavating the impacted area and loading the material into a Diaz operated dump truck. The material will be transported via rail to Tunnel Hill Reclamation in Ohio. The dimensions of the excavation area of the impacted material was determined to be 20’X25”X6’.

I took a walk around the location and everything looked OK.

The inspection log indicated that Mike Demming conducted a weekly/post weather inspection on 3/8/17. Mr. Deming and Lucas Schwarztrauber(Measurement Technician -SWN) were present during the inspection.

The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge and a 5000psi pressure gauge with 0psi on it. There was 306psi on the production casing and 266psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 11:45a.m.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2741122 Primary Facility 2018-06-13 Routine/Complete Inspection No Violations Noted I Inspected the Herman pad. The wells associated with this pad are the 115-20568, 115-20570, 115-20572, 115-22097, 115-22098. The wells were labeled.

The pad surface and access road were both solid and stable. The cut and fill slopes were well vegetated and had no erosional issues noted.

There were no violations noted.

2825709 Primary Facility 2019-01-11 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel and I was unable to check for gas at the perimeter of the cellar because the gravel was frozen. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2845270 Primary Facility 2019-02-27 Routine/Complete Inspection No Violations Noted I performed a site inspection of the Herman well pad on February 27, 2019. The weather was snowing lightly and in the mid 20s. I was previously at a water supply complaint in the vicinity. There was a water hauler truck broken down on the small access road. No leaks were observed.

The access road and pad surface were frozen hard, as were all slopes. The vegetation from the previous growing season was abundant on all areas. Woody vegetation was establishing itself in some areas as well.

There were stone piles and a skid steer loader onsite but not active. There were no areas of leaks or drips observed.

The wells were all well labeled.

There were no erosional issues noted.
