2093154 Primary Facility 2012-08-17 Routine/Complete Inspection No Violations Noted Operator has requested to backfill the cellar of well. Proper well tag is in place on well head. The cellar appears clean. At time of inspection there appeared to be no issues of concern. No violations observed
2110170 Primary Facility 2012-10-11 Follow-up Inspection Outstanding Violations - No Viols Req'd On 10/11/12 I arrived on site and met with WPX Energy to re-inspect the well. Based on previous inspections, evidence of gas bubbling was observed at the surface in the 9 5/8" x 13 3/8" annulus. A violation of the Department's regulations, 25 PA Code 78.86 was cited for this well based on the existence of bubbling in a DEP inspection report dated 9/13/11 (Inspection Record #2002321). At the time of this inspection, there was minor bubbling present in the annulus. There are four wells drilled and fracced on site (1H, 3H, 5H, and 7H). All four wells are online for production. Carty 3H and 5H were temporarily shut in during this inspection. No new violations were observed while on site. An outstanding violation of 78.86 is still existing. The Operator shall forrect the defect or submit a plan to correct the defect for approval by the Department within 30 days of receipt of this inspection report.

2427913 Primary Facility 2015-11-25 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:35 a.m., the cellar had water at the bottom, the (13x9) vent pipe was open and venting into the atmosphere and there was no gas detected, the (9x5) vent pipe was open and set up with a Taylor plug and with a 1000psi pressure gauge that had about 35psi on it, there was 335psi on the production casing and 259psi on the production line. I took a walk around the location and everything looked OK.
2508860 Primary Facility 2016-05-09 Routine/Complete Inspection No Violations Noted Inspection of the Carty well pad in conjunction with the Northeast Gathering Pipeline - Line 408 pipeline notice of termination inspection. There were no issues observed on the well pad at the time of the inspection.
2524706 Primary Facility 2016-09-29 Routine/Complete Inspection Outstanding Violations - No Viols Req'd At time of inspection, arrived on site at 10:05a.m., the cellar was about a quarter full of water with no bubbling noticed. There was a cap on the (13x9) annulus with a vent line and was venting into the atmosphere, there was no gas detected. The (9x5) vent pipe was open and set up with a Taylor plug to check for pressure with a 1000psi pressure gauge that was showing about 30psi on it. There was 217psi on the production casing and 165psi on the production line. I took a walk around the location and there were no obvious concerns noticed.

Violation 25 PA Code 78.86 was cited for this well in an inspection report dated 9/13/2011 (inspection record 2002321). I left the site at 10:45a.m.

The cellar was full of water with no bubbling noticed. The (13x9) annulus vent pipe was open and below the water level with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge and a 1000psi pressure gauge with about 20psi on it. There was 286psi on the production casing and 282psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 10:40a.m.

2625269 Primary Facility 2017-08-11 Routine/Complete Inspection No Violations Noted The Department arrived on site at the WR 77- Carty Well Site to conduct a Routine Complete Inspection.
