DEP will follow up.

I took a walk around the location and everything looked OK.

At time of inspection, arrived on site at 1:35 p.m., the cellar had water at the bottom with no bubbling noticed, there was cement between the (20x13) and (13x9) annuli’s with no gas detected, there were no lines going to or from the well. I met with SWN well tender Jack Padula, Jack used a Crystal pressure gauge to get the pressures off the (9x5) annulus and 5.5 production casing, there was 132psi on the (9x5) annuli and 0psi on the production casing. There were three other wells on site (115-20576 1H), (115-20578 3H), (115-20580 5H) that were in production. There were no obvious concerns noticed at time of inspection. I left the site at 2:55 p.m.

There were no issues observed on the well pad at the time of the inspection.

There were no lines going to or from the well. The well was granted inactive stasis on 10/21/16. I took a walk around the location and there were no obvious concerns noticed. I left the site at 10:40a.m.

2625274 Primary Facility 2017-08-11 Routine/Complete Inspection No Viols - Fac Inactive for Inspection The Department arrived on site at the WR 77- Carty Well Site to conduct a Routine Complete Inspection. A review of the E&S Inspection Log indicated that the most recent inspection was conducted on 8/7/17 with the note "monitor rills".

The status of this well is "Regulatory Inactive Status".

The well was granted regulatory inactive status on 10/21/16. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (13x9) annuli were below the water level and there was no bubbling noticed. There were no lines going to or from the well. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

The well was shut in and was granted regulatory inactive status on 10/21/16. There were no lines going to or from the well. The cellar was set up with a plastic corrugated pipe around the well for inspection access. The cellar was full of ice. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
