I took a walk around the location and there were no obvious concerns noticed.

The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge. There was 338psi on the production casing and 248psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:45p.m.

The well was in production. The cellar was full of gravel with water setting on top and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2760271 Primary Facility 2018-07-30 Routine/Complete Inspection No Violations Noted I inspected the Mitchell well pad on 07/30/2018 in response to a spill notification.

I met with company man Tommy Howard on site to perform an inspection. There apparently had been communication between the well bore and the mousehole which was discovered when a righand stepped on the plastic surrounding the hole and discovered it was puffed up with water. The water went back down shortly thereafter and there was no residual water after that initial discovery. The company had just started pumping cement and was just getting returns when it was discovered. The company is currently drilling on the Mitchell 1h. When the incident was noticed they had been running 13 3/8" surface casing which was just finished being flushed out with fresh water.


When the rig moves off, the site will be checked out after the plastic is removed and screened for contamination according to the company man.

The cut and fill slopes are well vegetated and stable.

2834151 Primary Facility 2019-01-30 Incident- Response to Accident or Event Violation(s) Noted WQS Gene Rickard and I performed an INCDT inspection of the Mitchell well site regarding a reported release which occurred aroung 0830 on Jan. 30, 2019. The complaint number was 339494. The release was reported to me via Briana Cunningham, WQSS. Other individuals on site were Mike Deming and Mr. Otten of Southwestern Energy Production Co.

The weather at the time of inspection was around 15 degrees F and snowing. Wind was gusting.

At about 0830 hrs. a tank in the onsite tank battery was being filled from a production fluid tank. The operator allegedly was monitoring the tank level from the hatch nearest the front part of the tank. According to a relayed assessment, the operator thought the tank was positioned not quite level, allowing the fluid level to overflow the back part of the tank, spilling onto the containment area surrounding the battery. The containment was later found to be compromised and allowed water to leave the containment area. SWN notified the Department on 1/30/2019. According to o

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar with a MSA Altair 5X multi gas detector that was calibrated on 1/2/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
