2485521 Primary Facility 2016-05-18 Routine/Complete Inspection No Violations Noted All wells listed on this inspection report have been drilled and stimulated and appear to be in production. Pad is stable. All slopes appear to be well vegetated and/or rock stabilized. No violations noted.
2497449 Primary Facility 2016-06-30 Routine/Complete Inspection No Violations Noted
2540084 Primary Facility 2016-11-22 Routine/Complete Inspection No Violations Noted On 11/22/16, the Department conducted an inspection of the SWN RU 18 Lyncott Corp Pad as part of an investigation into a complaint that a nearby water supply may have been affected by natural gas drilling operations. During the inspection it appeared as if all 7 wells (1H, 2H, 3H, 4H, 5H, 6H, and 7H) present were in production. The last well drilled and stimulated on this site is the 7H with drilling having been completed on 2/3/14 and stimulation having been completed on 5/31/14. In general, the pad appeared to be stable with no violations noted.
2576718 Primary Facility 2017-03-27 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:20a.m.

The cellar was full of gravel and there was containment around the well, the containment appeared to be in good condition. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 672psi on the production casing and 572psi on the production line. There was a 500bbl flow back tank on containment and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. I left the site at 11:15a.m.

2674576 Primary Facility 2017-12-12 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was shut in at the main valve after the GPU where the line comes out and goes to the gas line. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2771178 Primary Facility 2018-09-04 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
