TOH and set two cement plugs (each 500') at 4830 - 5330' and 4330 - 4830'. Company Man indicated the cause of loss circulation was not determined. At the time of inspection were drilling ahead at 5329. BOP charts were posted. Walked site. Made not about oil-base mud spray off of shkers. Operator indicated they are working on improving this system. Spray observed contained on plastic. Driller log on site and up to date. Containment in good condition. No violations observed at time of this inspection.
2093180 Primary Facility 2012-08-17 Routine/Complete Inspection No Violations Noted Operator has requested to backfill the cellar around base of well. Proper well tag is in placeon well head. The cellar appears clean. At time of inspection there appeared to be no issues of concern. No violations observed.
2343341 Primary Facility 2014-12-18 Routine/Partial Inspection No Violations Noted The Department was on site at the Knapik well site located at 2461 Valley View Road Montrose, PA 18801 to conduct a Routine Partial Inspection. The E&S Inspection log showed that Cassidy Strickland conducted an inspection on 12/12/14 and that the site was in “good condition”. The inspection previous was dated 12/02/14. Operator, please be advised that your inspection frequency does not meet the minimum requirements in 102.4(b)(5)(x). At the time of the inspection, the majority of the site was snow covered. No obvious E&S issues were found.

2425006 Primary Facility 2015-11-16 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:30 p.m., the cellar was full of gravel, the vent pipe was open and set up with a Taylor plug, there was no gas detected at the perimeter of the cellar, there was 343psi on the production casing and 273psi on the production line. I took a walk around the location and everything looked OK.

The vent pipe was open and set up with a Taylor plug to check for pressure and was also connected to the production line, there was 253psi on the production casing and 172psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 11:20a.m.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2821962 Primary Facility 2019-01-03 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was shut in at the GPU. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar with a MSA Altair 5X multi gas detector that was calibrated on 1/2/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
