2100333 Primary Facility 2012-08-16 Incident- Response to Accident or Event No Violations Noted Arrived at the site at 11:55 in response to a flowback spill that occurred at 8:45 PM on 8/14/2012. Scott Aharonian, EHS Advisor of WPX reported the spill to the Department at 8:00 AM on 8/15/12. The report indicates (15) gallons of freshwater/treated flowback was spilled from a frac tank onto secondary containment and cleaned up immediately. There were several areas of standing water on and off the secondary containment liner. During the inspection, I used an an Extech, ExStik (2) pH, Conductivity, Salinty and TDS meter to field screen these areas of standing water. The meter recorded (6) milliSiemens/cm between one of the wells and grate covering it and (5) milliSiemens/cm in ponded water on containment a short distance away from the wells. Also, several areas of the liner expereinced compromised integrity. However, there was no evidence that any spilled material contacted the surface of the ground on the Well Pad.

I took a walk around the location and everything looked OK.

There were no issues observed on the well pad at the time of the inspection.
2519036 Primary Facility 2016-09-14 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:50p.m., the cellar was full of gravel and no gas was detected. The vent pipe was open and set up with a Taylor plug to check for pressure, there was 211psi on the production casing and 169psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 13:30p.m.

2615744 Primary Facility 2017-07-13 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:25p.m.

The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge. There was 286psi on the production casing and 281psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:05p.m.

2636634 Primary Facility 2017-08-11 Routine/Complete Inspection No Violations Noted The Department was on site at the Robinson Well Site to conduct a Routine Complete Inspection. Currently the 5H, 6H, and 7 H wells are present. A 1550 dark green tank, labeled water only, and two pumps are hooked up to the wells. The containment around this tank has liquid in it with a rainbow sheen. Operator, please maintain this area to ensure compliance. Additionally, please submit documentation to the Department concerning the product stored in the tank, the material compatibility of the tank regarding the product stored, and the origin of the sheen.

Update: I later spoke to Mr. Deming who explained that water is being stored in the tank and that the sheen is originating from the pumps located within the containment. SWN will be vacuuming up the fluid and disposing of it. They are also discussing options concerning the leaking pumps and how to eliminate the release of oil to containment.

2719539 Primary Facility 2018-04-18 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. There was a 1550 gallon tank on containment and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2823024 Primary Facility 2019-01-07 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was shut in at the GPU. The cellar was full of gravel and was unable to check for gas at the perimeter of the cellar because the gravel was frozen. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
