Cement on 13 3/8" fell insitialy @ 35' from top of conductor. Topped off with 8 bbls - cement held. Used 30' of 1" tremie pipe. WOC 8 hrs prior to top job. Checked cellar - no issues observed. Walked site with Co. Man. secondary containment in place and in good condition. Drillers log ons ite and up to date. No violations observed at this time.
2074409 Primary Facility 2012-05-22 Follow-up Inspection Viol(s) Noted & Immediately Corrected Follow up on report of spill of dry cement on pad. Spill occured @ 10 pm on 5/21/12 when hose connection failed. Approximately 30 sks cement released on pad while cementing long string. At time of inspection Operator scraping up cement from pad (approximately 10 - 15 yds of pad material removed and disposed of. Walked site and observed small spill of SOBM on pad. Operator immediately cleaned up spill...less than 1 gallon. I inspected both areas after clean up. No further remediation required at this time. Violation immediately corrected.
2101017 Primary Facility 2012-09-19 Routine/Complete Inspection No Violations Noted A service rig operated by Mercer Well Service was on-site during the inspection performing a clean out of the well in preparation for a toe perforation which may commence on 9/20/12. R&T Oilfield Services was in the process of filtering fluid circulating through the well associated with the cleanout operation. During the inspection, I observed some areas of the secondary containment liner experiencing compromised integrity and several light towers housed on the surface of the ground on the Well Pad. WPX personnel informed me they would address the liner integrity and move the light towers onto secondary containment.

2326279 Primary Facility 2014-11-24 Routine/Partial Inspection No Violations Noted The Department was on site at the Carty-Wiseman well site located at 377 Shikellamy Trail Hallstead, PA 18822. At the time of the inspection, the Site ID Sign was installed and the E&S Plan was on site. The last five (5) E&S Inspection reports were from 10/16/14, 10/24/14, 10/30/14, 11/4/14, 11/14/14. Operator, please be advised that at this time, you are not meeting the minimum requirements stated in 102.4(b)(5)(x). Should the Department discover that this recurs, it may result in a Notice of Violation.

2429513 Primary Facility 2015-12-03 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:40 a.m., the cellar was full of gravel, the vent pipe was open and set up with a Taylor plug, there was no gas detected at the perimeter of the cellar, there was 361psi on the production casing and 332psi on the production line. I took a walk around the location and everything looked OK.

The vent pipe was open and set up with a Taylor plug to check for pressure, there was 205psi on the production casing and 177psi on the production line. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 10:00a.m.

2529621 Primary Facility 2016-10-14 Routine/Complete Inspection No Violations Noted Inspection in conjunction with the notice of termination inspection of the NE Gathering Pipeline 408. No E&S issues or spills noticed on the pad at the time of the inspection.
2543834 Primary Facility 2016-12-12 Site Restoration No Violations Noted The Department was on site with Seth Tratthen and Rich Gulyas (SWN) to conduct a Restoration Inspection in response to the submittal of a Notice of Termination (NOT). The E&S Inspection Checklist indicated that S. Moyer conducted the most recent inspection on 12/7/2016 and that no attention was needed. There was partial snow cover during this inspection. The Site Restoration plan map depicted an area NW of the wells that was to be restored, but the site conditions do not appear to have changed.

I recommend that this NOT be denied

2617557 Primary Facility 2017-07-19 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:40a.m.

The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge. There was 341psi on the production casing and 338psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 11:25a.m.

2722809 Primary Facility 2018-04-25 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2768453 Primary Facility 2018-08-28 Routine/Complete Inspection No Violations Noted I inspected the Carty Wiseman pad in response to a call from SWN informing the Department of the containment being torn for the produced water tank. The felt was torn but the inner plastic appeared competent with one small repaired area. There was no staining or evidence of contamination outside the containment area.

There were no erosional issues noted and the cut and fill slopes were well vegetated. The access road looks t have been recently regraded and rolled as well.

There were no violations noted.

2825130 Primary Facility 2019-01-10 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was shut in at the GPU. The cellar was full of gravel and I was unable to check for gas at the perimeter of the cellar because the gravel was frozen. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
