2530528 Primary Facility 2016-10-19 Routine/Complete Inspection No Violations Noted Pre-plugging inspection of SWN’s Shively Sorenson well in Lenox Twp. Susquehanna County conducted October 19, 2016 at 10:45 am. Operator has sent Notice of Intent and plugging prognosis for review. At the time of inspection; I met with SWN on location. The 2H has a production tree in place, shut-in, not tied into pipeline. Analog pressure gauge at the tree top (tubing) and wing valve (prod. csg.) both show approx. 3300 PSI, verified with SWN well tender’s crystal gauge at taylor plug valves. The Crystal Gauge broke when the well tender went to check the pressure on the backside of the production casing (“9 x 5”) annulus, so he opened up the valve on the otherside of the 9 5/8” casinghead up to a fluid-filled gauge; 0 PSI on the backside of production casing. I asked him for SWN to contact me if there is any pressure on the annulus after verifying with a different crystal gauge later in the day. The cellar was empty. The 9 5/8” csg’s annulus had water on the cement top; no bubbling or gas verified with Altair
2534422 Primary Facility 2016-11-03 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of SWN’s Shively Sorenson 2H in Lenox Twp. Susquehanna County conducted November 3rd, 2016 at 1 pm. CWS workover rig was staged on the pad for P&A operations. At the time of inspection; slickline was rigged up on the well with tools stuck in the hole. Well kill fluid being pumped. Once tools are recovered the rig will continue P&A procedure; pull tubing, set CIBP and set balanced cmt plug in days to follow. I plan to return after the rig pulls production casing and sets balanced cement plugs back from the stub-openhole and in openhole – back across 9 5/8” shoe. No major concerns at this time.
2535459 Primary Facility 2016-11-09 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of SWN’s Shively Sorenson 2H in Lenox Twp. Susquehanna County conducted November 9rd, 2016 at 10:30 am. CWS workover rig was rigged up on the well for P&A operations. O-TEX Cementers were spotting in and rigging up. I met with the Companyman to discuss operations on the pad since my last visit 11/3. CIBP was set at 7202’, balanced cmt plug set on top; the rig tagged plug at 7015’. Stretch test indicated estimated TOC @ 3200’. 1st 5 ½” csg cut was made at 2952’, pulled 60K over w/ no success, made 2nd cut at 2492’ and pulled casing. We went over depths and volumes for remaining balanced cement plugs. O-TEX rigged up to circulate mud out the hole w/gel and cement. No Issues.
2537569 Primary Facility 2016-11-15 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of SWN’s Shively Sorenson 2H in Lenox Twp. Susquehanna County conducted November 15, 2016 at 11:40 am to conduct inspection of adequacy of plugging after last week’s plugging operations. At the time of inspection, the casinghead on the 9 5/8” had marker cap welded flush with flange. In addition; Nipple/ ball valves have been removed from the side outlets welded closed, disallowing access to the wellbore before burial. Cap was welded approx. 1 day after mixing, pumping, and spotting production csg stub isolation plug and the 9 5/8” shoe plug, and approx. 2 hours after mixing, pumping, and spotting last (2) balance cement plugs inside the 9 5/8”. I contacted the operator in phone conversation from the pad to discuss the obvious problems with this practice. Operator plans to cut the abandonment cap to allow access to the wellbore in order to evaluate the adequacy of plugging to verify the job was successful. No violations cited at this moment.
2539816 Primary Facility 2016-11-21 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of SWN’s Shively Sorenson 2H in Lenox Twp. Susquehanna County conducted November 21, 2016 at 10:55 am. At time of inspection; 2” pipe nipple tapped into well abandonment cap w/valve, 2”x 1/2” reducer coupling, needle valve. I opened the 2” valve and needle valve; needle valve was full of ice. Shut in the 2”, removed the needle valve, opened the 2”, pipe nipple had obstruction inside where it threaded onto the marker cap. I poked through obstruction in nipple with steel rod and took a field reading with an Altair 5x gas meter which yielded 6% LEL. I installed a 0-150PSI analog pressure gauge on the well; no buildup after ~10 min. I then opened the bleeder on my pressure gauge to a balloon; no inflation, indicating no measurable flow of gas at this time. Although there is not a measurable flow of gas to warrant a violation at this time I am recommending the operator let the well open to allow any residual gas that may have been trapped between surface plug and marker cap to escape and to make sure t
2539821 Primary Facility 2016-11-29 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of SWN’s Shively Sorenson 2H in Lenox Twp. Susquehanna County conducted November 29, 2016 at 12:00 pm. At the time of inspection; the 2” valve and needle valve coming off the marker cap were open, having been vented since my last site visit 11/21 per my recommendation to the operator. I removed the needle valve and threaded ½” pex fitting w/hose attached to Altair 5x gas meter. A field reading with the gas meter yielded no gas detectable in any amount at this time. I gave the operator the go ahead to shut in and bury the well. No violations noted.
2558463 Primary Facility 2017-02-01 Plugging(Includes Plugged/Mined Through) No Violations Noted Routine inspection of a plugged well. The well was plugged ~3 months ago. The below-ground marker is now buried in the cellar with gravel. The location was snow covered and there were no obvious concerns noted on the surface.
2852643 Primary Facility 2019-03-08 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine, post-plugging inspection of this plugged unconventional well was conducted March 8, 2019. At the time of inspection, the site is reclaimed and the plugged well is now cut and buried below grade. The location was snow-covered. I stood at the buried well location; there were no open holes, discharges, venting, staining, bubbling, or other obvious issues observed at this time.
