Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2129789 Primary Facility 2013-01-03 Drilling/Alteration No Violations Noted At time of inspection no activity. Gate locked to access road and road no plowed. Well has been spudded. Will return when have better access to pad. No violations.
2287692 Primary Facility 2014-07-14 Routine/Complete Inspection No Violations Noted Falcon Rig# 18 is the active drilling rig on the location. Laddie Pendergraft is the company man on location. The current drilling depth is (3,543) feet. According to Mr. Pendergraft, the vertical section of the Humbert 5H well is being drilled through the process of air drilling. The surface casing was cemented at (613) feet and the intermediate casing was cemented at (1,641). Falcon Rig #18 will drill the vertical section of the well to a total depth of (5,365) feet. This is set to be completed tomorrow morning, 7/15/14. The Humbert 1H well is in the production phase of operations. Fluid level in tank to contain drill cuttings is right at 2' freeboard level. Drilling rig secondary berm behing brine box used for solids control is uneven and not completely secure.

2565642 Primary Facility 2017-02-27 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:00p.m.

The cellar was full of gravel with water setting on top and there was no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure and the valve going to the production tank was closed. There was 502psi on the production casing and 354psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:55p.m.

2663879 Primary Facility 2017-11-22 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2732621 Primary Facility 2018-05-21 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. No E&S issues were observed. No violations noted.

2774289 Primary Facility 2018-08-22 Routine/Complete Inspection No Violations Noted The Department arrived on site at 1417 hours to conduct a RTNC inspection. Present to represent the Department were Gene Rickard (WQS) and myself.

The pad surface and access road were stabilized with no apparent erosional issues. All the slopes and berms were well-vegetated and stabilized.

The produced fluid tank and its containment appeared to be in good condition. The GPUs and their containment also appeared to be in good condition, although the containment was about halfway full with stormwater and should be vacuumed out.

The well heads were properly labeled with the name and permit number. No visible bubbling was visible in the well cellars. The entrance to the access road was marked by the required signage. All required paperwork was present in the pad's mailbox.

The Department concluded the inspection and left the site at 1438 hours.
