2110109 Primary Facility 2012-10-10 Drilling/Alteration No Violations Noted Polaris - Orion Rig. Drilling ahead on air at depth of 993'. Walked site with Co. Man Dave Sheffield. Secondary containment in place and in good condition. Will be runing 9 5/8" on 10/11 and cementing. Using 'superflush" now on cement jobs on surface and intermediate casing strings to help eliminate cement fall back. So far this has been successful. Appears to clean and condition hole very well which allow better bond for cement and eliminates air pockets which were causing cement fall back on previous jobs. No violations observed at this time.

2502178 Primary Facility 2016-07-14 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:20p.m., the cellar was full of gravel and no gas was detected. The (9x5) vent pipe was open and set up with a Taylor plug, there was 132psi on the production casing and 109psi on the production line. I took a walk around the location and everything looked OK.

Floyd Parrish of SWN was on site during my inspection. I left the site at 14:10p.m.

2593777 Primary Facility 2017-05-10 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:45p.m.

The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge. There was 183psi on the production casing and 96psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:20p.m.
