I took a walk around the location, everything looked OK.

There were no issues observed on the well pad at the time of the inspection.
2513779 Primary Facility 2016-08-24 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:15a.m., the cellar was full of gravel and no gas was detected. The vent pipe was open and set up with a Taylor plug to check for pressure, there was 177psi on the production casing and 167psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 11:55a.m.

2605615 Primary Facility 2017-06-19 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:40p.m.

The cellar was full of gravel with water setting on top with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge and a 5000psi pressure gauge with about 55psi on it. There was 219psi on the production casing and 178psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:30p.m.

2607857 Primary Facility 2017-06-27 Routine/Complete Inspection No Violations Noted I inspected the site on June 1, 2017 after receiving notification from SWN that the site was scheduled to have plugging operations performed on the wells. The well pad was solid, but had some puddles from recent rains. There was a spoil pile in the middle of the well pad. The cut slope had evidence of past repair but repair was stable. There was no apparent sediment leaving the site after recent rains and all slopes were well vegetated. The well cellars were filled in so bubbling could not be observed. The access road was solid and stable with no apparent washing.

There were no erosional issues observed during the inspection. There were no violations noted.

2713004 Primary Facility 2018-03-28 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel with water setting on top and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2814768 Primary Facility 2018-12-13 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was shut in at the GPU. The cellar was full of gravel and was unable to check for gas at the perimeter of the cellar because the gravel was frozen. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
