I took a walk around the location, everything looked OK.

There were no issues observed on the well pad at the time of the inspection.
2519027 Primary Facility 2016-09-13 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:15p.m., the cellar was full of gravel and no gas was detected. The vent pipe was open and set up with a Taylor plug to check for pressure, there was 317psi on the production casing and 189psi on the production line. I took a walk around the location and everything looked OK.

I left the site at 13:45p.m.

2613949 Primary Facility 2017-07-07 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:05p.m.

The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge. There was 258psi on the production casing and 180psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:40p.m.

2716366 Primary Facility 2018-04-09 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. I noticed the packing on both of the production valves were leaking gas and fluid. I contacted Steve Moyer of SWN to let him know about the valves and have him contact the Department when they were repaired. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2818073 Primary Facility 2018-12-20 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was shut in at the GPU. The cellar was full of gravel and was unable to check for gas at the perimeter of the cellar because the gravel was frozen. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
