2327405 Primary Facility 2014-12-04 Routine/Complete Inspection No Violations Noted Inspection conducted on 12/4/14 of the SWN RU 70 Loke Well Pad. There is one well in production on this pad, the Loke 2H (115-21077). The inspection was conducted to review a small section of the pad slope which has been destabilized due to equipment traversing this section by the adjacent quarry operation. Stone has been added to this section, but is not adequate in completely stabilizing this section. A wood chip filter berm is located immediately down gradient of the disturbed area and appears as if it can temporarily handle sediment from this location. The Department recommends stabilizing this section completely with stone. The Department will inspect this section in 2 weeks for compliance.

The cellar was full of gravel with water setting on top and there was no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure and the valve going to the production tank was closed. There was 639psi on the production casing and 382psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:55p.m.

2604959 Primary Facility 2017-06-09 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected I met with Mike Deming, Senior Regulatory Analyst and Seth Tratthen, Regulatory Analyst of SWN shortly upon arrival to the site. Mr. Deming and Mr. Tratthen stated the following. "Southwestern Drilling Company Rig #45 is in the progress of rigging up. The company man on duty is Jonathan Woodburn. Rig up commenced on 6-7-17. The 4H well will be drilled to completion. This well was drilled to kickoff point by Well Services Group Rig # 1. SDC 45 will re-enter the well. A Caterpillar generator spilled (150) gallons of diesel fuel to containment. It is believed that the impact to the pad surface in the general area where the spill occurred is less than (10) gallons. The incident is still under investigation. A seal in the fuel transfer pump for the generator failed. It was approximately a one quarter inch weep hole that the fuel leaked through. A gasket inside of the transfer pump failed potentially due to over-pressuring or a crack. It has not been determined as to how long the pump was leaking for. There were
