2177123 Primary Facility 2013-05-21 Routine/Complete Inspection No Violations Noted The primary reason for my inspection was to view the Aerion Rental Sevices centrifuge system. This system, which was first encountered by the Department at the Range Unit 64 (Boman) Well Pad on 5-10-13, separates solid and liquid material from drill cuttings during air and mist drilling of the vertical section of the well. Solids are deposited into steel containers in preparation for landfill disposal and the remaining liquid goes into frac tanks labeled dirty water. The liquid material, which is essentially a fluid, is beneficially reused for dust control during air and mist drilling. This process involves the addition of chemicals consisting of calcium nitrate and a polymer as a slurry feed for flocculation. An alternative waste management practice (OG-71) authorization is required for this process. On May 31, 2013 SAIC forwarded to me via e-mail a revised OG-71 template for this site. The template contained MSD sheets for SEDIFLOC 7115, an anionic organic flocculant and synthetic organic polymer.
2467800 Primary Facility 2016-03-21 Routine/Complete Inspection No Violations Noted On 3/21/16, the Department conducted an inspection of the SWN RU 29 Zeffer Pad and the 2 impoundments. The small impoundment was inspected first. The new liner has been installed. Normal operations are now occurring. During the inspection 2 water trucks were observed off-loading to this impoundment. All equipment associated with the dewatering and solids removal of the small impoundment has been removed. As suggested by the Department, the soil has been removed and replaced from the area between the off-loading concrete pad and the small impoundment. The disturbed areas has been seeded and mulched. The small impoundment appears to be about 1/3 full and the large impoundment appears to be approximately 1/3 full. All of the slopes associated with the pad and impoundments were inspected and appeared stable and well vegetated. No violations noted.

2509157 Primary Facility 2016-08-03 Routine/Complete Inspection No Violations Noted On 8/3/16, the Department conducted an inspection of the SWN Zeffer Well Pad and Impoundment’s 1 and 2. Also present during the inspection were Eric Rooney, Department Geologist and Mike Deming, SWN Senior Regulatory Analyst. Mr. Rooney was on-site evaluating SWN’s efforts in meeting their Work Plan submitted by CB&I consulting dated March 10, 2016, titled in part as “Additional Hydrogeologic Investigation Report for the Zeffer Impoundments.” At the time of the inspection, a small drill rig was on-site collecting core samples next to Monitoring Well # 8. This activity is being conducted to ensure that the monitoring well was constructed and installed properly based on the localized geology. A review of the large and small impoundment found that they were below the 2’ free board level required of waste water impoundments. The large impoundment had a free board level of about 4’, while the small impoundment was in the range of 5’. A review of the well pad found that there is good vegetative stability, a
2555318 Primary Facility 2017-01-25 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:05p.m., the cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The vent pipe was open and set up with a Taylor plug to check for pressure and the valve going to the production tank was closed. There was 661psi on the production casing and 566psi on the production line. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 13:50p.m.

2693651 Primary Facility 2018-02-09 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. No E&S issues noted. No violations noted.
2736057 Primary Facility 2018-05-23 Routine/Complete Inspection No Violations Noted The department arrived on sited at 1220 hrs to conduct a RTNC inspection of the Zeffer 29 Well Pad. The department was represented by Casey Baldwin (DEP-WQS) and myself.

Upon arrival at the pad, it was noted that the proper signage was displayed at the entrance to the pad's access road, and the appropriate paperwork was present in the mailbox. The access road appeared solid and stable, with no apparent erosion even after substantial rains during the past few days.

No E&S issues were observed, and all PCSM stormwater management BMPs were present and in good condition.

Mr. Baldwin and I departed the pad at 1320 hrs.

2751711 Primary Facility 2018-07-13 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2844414 Primary Facility 2019-02-22 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. As the photo's show, the site is snow covered. Overall the site appears to have good stabilization. No E&S issues identified. No violations noted.
2852346 Primary Facility 2019-03-12 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar with a MSA Altair 5X multi gas detector that was calibrated on 3/4/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
