2234003 Primary Facility 2013-12-11 Preoperation Inspection No Violations Noted The Department was on site at the SWN Range Unit 50 Mid Well Site to conduct a Preoperational Inspection. Inspection conducted with Rich Gulyas and Seth Tratthen. Well permit that has been issued for this pad is the Sheldon East 4H (115-21114). All perimeter BMP’s in place to include red lined additions. Pad is 90% complete. Revised plan of site has a smaller pad due to reduced number of proposed wells. Pad is snow covered. Seth states that the pad slopes are approximately 80% rock stabilized. Unable to confirm due to the snow. 2 level spreaders associated with access road cross culverts are borderline meeting the minimum construction standards. Seth will watch these spreaders and address if issues do arise. Unable to assess level spreader for access road cross culvert due to snow cover. No violations noted.

2254838 Primary Facility 2014-03-11 Drilling/Alteration No Violations Noted SDC Rig #10 is the active drilling rig at the location. The current drilling depth is (1,615) feet. Mr. Herbert stated that Southwestern Energy is operating under due diligence in lieu of the expired permit. While in the process of surveying the secondary containment systems on the location, the backyard and solids control process, I screened water for conductivity on the drilling rig secondary containment system in between the roll-off containers in the backyard and the enclosed oil based mud and floc water tanks. These tanks are covered with a plastic liner and there is a heating system used to prevent the fluid in the tanks from freezing. Conductivity readings ranged between (1,200-1,500) microSiemens/cm. An employee from Environmental Drilling Solutions was pressure washing equipment associated with the solids control and drilling mud dewatering process. It appeared as if the wash water flowed over the wooden rig mats in the backyard, onto the (40) mil Western liner.

The well was in production. The cellar was full of gravel, unable to check for gas at the perimeter of the cellar because the gravel was frozen. The (20x13) and (13x9) annuli were below the gravel. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
