2356696 Primary Facility 2015-02-27 Routine/Complete Inspection No Violations Noted Falcon Rig #27 is currently drilling the King South 5H well @ (1,620) feet. Bud Woolsey and Derreck Perry are the company men on location. The intermediate casing was set @ (1,620) feet, the surface casing was set @ (727) feet and the conductor @ (62) feet according to Mr. Woolsey. Mr. Woolsey stated that Falcon 27 will drill to (5,650) feet which is (100) feet above the kick off point. The well will be filled with oil based mud upon completion of drilling to the area above the kickoff point. A re-entry rig will drill the horizontal section of the well at a date to be determined. The King North 3H well is in production.
2358447 Primary Facility 2015-04-06 Incident- Response to Accident or Event No Violations Noted A review of the overall containment for the drilling rig found the containment to be of excellent construction, which would lead one to surmise that the primary containment below the matting at the spill location to be intact. Please contact the Department when the Rig moves off and the rig mats are removed so that the Department can verify that the primary containment at the spill location has not been compromised and that a release to the ground has not occurred.
2361850 Primary Facility 2015-04-16 Routine/Complete Inspection No Violations Noted
2366958 Primary Facility 2015-05-04 Routine/Complete Inspection No Violations Noted Based on the unknown volume of the release and the visual documentation collected, this case will be referred to the Department’s EC&B Program. The violations noted in this inspection will be assigned to the King South 6H, Inspection Record # 2366945 and Enforcement Record # 322862 .
2374697 Primary Facility 2015-06-01 Administrative/File Review No Violations Noted The attached letter is SWN's written response to the violations noted in the field inspection conducted on 5/4/2015. The violations are associated with the King South 6H, Inspection Record # 2366945 and Enforcement Record # 322862 .

2401029 Primary Facility 2015-08-18 Routine/Complete Inspection No Violations Noted Well in production. Cellar completely filled with gravel. 3 treaters and 1 tank on location. 603.47psi on 51/2
2578047 Primary Facility 2017-03-23 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete Inspection. At the time of the inspection, the E&S Inspection Checklist Site Inspection Log indicated that SRT conducted the most recent inspection on 3/17/17 and no attention needed.
2580425 Primary Facility 2017-04-06 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:15a.m.

There was a crew on site picking up the containment that was under the tanks and around the 7H, 8H, and 9H wells. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 858psi on the production casing and 753psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 10:10a.m.

2637596 Primary Facility 2017-08-14 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete Inspection. At the time of arrival, I reviewed the E&S Inspection Log which indicated that the most recent inspection was conducted on 8/7/11. On site, several 21,000 gallon tanks are situated on containment. Currently the King 9H, 7H, 8H, 5H, 6H, and 3H appear to be in production, while the 1H, 2H, and 4H are present but do not appear to be on line. The access road has sediment built up in areas along the edge, in the roadside ditches, culverts, and rip rap/sump areas. Sediment has also fanned out from the access road rip rap apron/sump. Operator, please maintain the features mentioned per your approved E&S Plan.
2658048 Primary Facility 2017-10-25 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete Inspection in response to the receipt of a Post Drilling Site Restoration Report.
2706249 Primary Facility 2018-03-14 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was temporarily shut in because it was loaded up with fluid when the were stimulating the 1H, 2H and 4H wells. The cellar was full of gravel and was unable to check for gas because the gravel was frozen. There were 25-500bbl water tanks and a 500bbl flow back tank on containment from a recent frac on the 1H, 2H and 4H wells, the containment was snow covered. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2809814 Primary Facility 2018-12-04 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
