A valve was not locked on the tank as it was in the process of being filled. A portion of the fluid escaped secondary containment and impacted the surface of the ground at the site. The impact to the ground surface is apparently due to compromised liner integrity and overflow of the fluid on containment to the ground surface. Compromised liner integrity was observed near the tank battery containment berm. I recorded elevated conductivity readings, over range on my meter on and off containment on the Pad and in an interceptor trench just the below the Pad dug to collect the fluid as a subsurface release is apparent. |

I took a walk around the location and noticed a role back container full of containment that was not on containment and not sealed. The pad only has the standard equipment associated with steady state production. No other activity is occurring at this pad. I spent approximately 5 minutes at each corner of the well pad and did not detect any malodors. No E&S issues were observed during this inspection. No violations noted. |

The conductors were all drilled to a depth of approximately 105'. During the 7/3/18 inspection, the inspector noted that 3 Rigs were being operated, one auger rig and two air rigs. No E&S issues were identified. No violations noted. |
