Well permits that have been issued for this pad are the 1H (115-21261), 2H (115-21262), 4H (115-21263), and the 6H (115-21264). Inspection conducted with George Bishop of GDS. All perimeter BMP’s in place. Pipeline is actively being installed across the access road approximately 600’ from the entrance. In process of hydroseeding the pad with approximately 65% complete. Pad is approximately 95% complete.

2507143 Primary Facility 2016-08-01 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:05p.m., the cellar was about three quarters full of water with no bubbling noticed, the (13x9) vent pipe was open and venting into a tank, the (9x5) vent pipe was open and venting into a tank, there was 513psi on the production casing. I took a walk around the location and noticed a role back container full of containment that was not on containment and not sealed. I contacted Steve Watson of Cabot Oil and Gas to let him know of the container and he told me he would get it taken care of.

I left the site at 13:45p.m.

2533319 Primary Facility 2016-11-01 Routine/Complete Inspection No Violations Noted On 11/1/16, the Department conducted an inspection of the COG Adams J Well Site located in Harford Township, Susquehanna County in response to a nearby malodor complaint. The Department arrived on-site at approximately 10:00 AM. There are 2 wells drilled and in production on this pad, the 4H and the 6H. The 1H and 2H were proposed but never built. The pad only has the standard equipment associated with steady state production. No other activity is occurring at this pad. I spent approximately 5 minutes at each corner of the well pad and did not detect any malodors. No E&S issues were observed during this inspection. No violations noted.
2605592 Primary Facility 2017-06-15 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:55p.m.

The cellar had water at the bottom with no bubbling noticed. The (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. There was 493psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:30p.m.

2709604 Primary Facility 2018-03-22 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was ice 4' below grade in the cellar and there was no gas detected from there to the surface. The (13x9) and (9x5) annuli vent pipes are connected together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2750984 Primary Facility 2018-07-09 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as part of an investigation into a nearby water supply complaint in which the water well was impacted by a heavy sediment load on 7/3/18. As Photo #2 shows, new wells have recently been spud on this pad. A review of the most recent Department Drilling Inspection, Inspection Record No. 2748434 found that conductor pipe was installed on 7/3/18 for 7 wells, the 1H, 2H, 3H, 5H, 7H, 8H, and the 10H. The conductors were all drilled to a depth of approximately 105'. During the 7/3/18 inspection, the inspector noted that 3 Rigs were being operated, one auger rig and two air rigs. No E&S issues were identified. No violations noted.
2850438 Primary Facility 2019-03-07 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. Present during the inspection was Cabot Site Supervisor Steve Gillman. Stimulation activities have been completed and site is currently being demobilized. Production equipment installation is also occurring. Overall the site appears to have good stabilization. No E&S issues identified. No violations noted.
