The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 761psi on the production casing and 651psi on the production line. Halliburton was on site and logging the 2H, 3H, and 5H wells. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:45p.m.

2591757 Primary Facility 2017-05-09 Routine/Complete Inspection No Violations Noted Inspection conducted on May 4, 2017. Minor damage shown in photo 2 needs to be repaired. The road side ditch from 5+00 to 10+00 needs to be reestablished to design specifications. Photo 4 shows damage near entrance to pad. This damage needs to be repaired. Appears as if this location is a low point on the pad which is causing the damage. Please have your pad designers review to determine if controls should be added at this location, possibly a level spreader. Conklin North 1H and Conklin South 5H appear to be in production. Other three wells drilled are the North 2H, South 5H, and the North 3H. Photo 5 shows garbage that has been left on the pad. Please remove, before a storm event does.
2684967 Primary Facility 2018-01-08 Incident- Response to Accident or Event No Violations Noted Gene Rickard, DEP O&G WQS and I arrived at the site at 11:29 in response to a reported spill of (300-400) gallons of production fluid to secondary containment, with (2-3) barrels of the fluid spilled escaping the containment to the pad surface. The weather conditions were cloudy skies, cold and breezy with temperatures in the 20's. US well service is the SWN subcontractor at the site in the process of conducting hydraulic fracturing (stimulation) operations. Mike Deming, Senior Regulatory Analyst and Zachary Evans, well site supervisor/site company man, both of SWN, accompanied the Department during the inspection. Dawn Washo represented Resource Environmental of Montrose, PA. Mr. Deming and Mr. Evans provided the following details regarding the spill and site activities. "We are on stage 41 of 43 for the frac on the Conklin South 5H well. We are pressured up and perforating. The 3H is the next well to be completed. A total of (300) to (400) barrels of production fluid was spilled to containment. There were
2688035 Primary Facility 2018-01-23 Incident- Response to Accident or Event No Violations Noted Today's inspection was conducted as an incident inspection. At approximately noon today, Mike Deming, SWN Senior Regulatory Analyst contacted the Department to inform the Department that SWN had identified stormwater discharges off secondary containment on 3 of their well pads, of which this pad is one. The discharge from containment occurred when ice formed and collapsed the containment edge. Photo 2 shows the containment edge being reestablished. The Department conducted conductivity metering and found that outside containment conductivity ranged between 150 uS and 250 uS. Inside containment readings ranged from 165 uS to 250 uS. Work over rig on site drilling out plugs. Currenty in 3H. Of note is the fact that a production fluid spill had occurred on January 5, 2018 in the photo 3 vicinity. No E&S violations were observed. No violations noted.

The battery of water tanks are still present and located on containment. All 5 wells appear to be in production. Photo 2 shows trash that needs to be removed. No violations noted.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar with a MSA Altair 5X multi gas detector that was calibrated on 1/2/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2841929 Primary Facility 2019-02-14 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. As the photo's show, the site is snow covered. Overall the site appears to have good stabilization. No E&S issues identified. No violations noted.
