At this time, the conductor was being drilled with a Soilmec SR30 (excavator based) rig. A Patterson 3 stack rig will be scheduled to arrive on site at a future date to complete any further drilling. There was adequate vegetation throughout the site, and there were no erosional issues apparent at this time. Talisman's last E&S inspection was on 3/29/2016. There are no violations. There was 633psi on the production casing and 393psi on the production tubing. I took a walk around the location and noticed dead vegetation on the right side of the site from the entrance, I contacted Frank Hoehle (DEP water quality specialist) and he informed me that he knew of the area.

After contact with Mr. Perritt, it was concluded that it was indeed concrete cleanout on the pad surface, which is to be cleaned up during upcoming activity at the pad. There was also vegetation killer sprayed on the pad surface itself, resulting in the dead vegetation observed during the visit.

"We are in the process of running a cement bond log. The intermediate casing will be run today and cemented later today. We are drilling with water based mud through the Upper Devonian formation. The surface casing was cemented at (562) feet. We will drill with water based mud through the kickoff point at (4,800) feet. Soli-bond is used to dry the cuttings instead of sawdust or wood pellets. I arrived at the site at 12:30 PM on 1/6/17 to conduct an investigation of the incident. I met with Tory Rogers, site safety man of REM security. Talisman was in the process of breaking down the Patterson number 805 drilling rig and equipment and moving it off-site. Mr. Rogers stated the following information associated with the spill. "The spill occurred yesterday, 1/5/17, around 9:00 AM. (2) gallons of synthetic oil based mud from a flow line from that runs from the rig to the mud tanks was spilled to the pad surface about 60 to 70 yards from the where the lease road and well pad meet. The pipe was loaded onto the back of a flatbed truck and rolled as it was set spilling the mud out of the pipe onto the ground. There was no bull plug on the end of the pipe. The spill was cleaned up immediately. A bull plug was attached to the pipe. |

"Bill Watts of Talisman notified Kevin Augustine (NERO - ER) who then referred spill to Eric Rooney (Scranton Office - O&G) that 170 gallons of hydraulic fluid was spilled onto 2nd containment." We met with Dave Coxhead, site safety man of REM security upon arrival to the location. The release occurred because the auto valve lock on the blender failed so the valve was then manually operated, allowing approximately 20 gallons of blended fluid to overflow the blender and escape onto secondary containment. The 500bbl water tanks that were used during stimulation were still on site and on containment, the containment appeared to be in good condition. The weather conditions were sunny skies with a light breeze, very warm and humid with temperatures in the mid to upper 80's. There was no activity or personnel at the site. All wells at the location are in production. Secondary containment utilized for completions operations has been removed from the site. During the inspection the Department performed specific conductance (electrical conductivity) screening of water on the pad surface, in well cellars, the production fluid tanks containment structure, the gas production units containment structure and frac tank containment structure. There were no elevated conductivity readings of concern. All readings recorded were less than (600) micro Siemens/cm. We were not able to locate a post storm Erosion and Sedimentation inspection report in the mailbox on the access road near the pad entrance. The Department left the site at 13:46. |

The man in charge on site representing Ensign was Jimmy White. There had been erosional issues and a spill on site previously. We were on site to conduct a follow up inspection for these issues. The acting company man for Ensign stated they were currently flowing back the 1h well. Jon took conductivity readings at various puddles around the site. Examples are as follows: .67 mS, 1.3, .30 in tank battery, .41, .67, .95, .5, .43, 1.7. Pad was dry during inspection. The flow back lines were not on containment but all appeared dry. 5mS in containment for gasbuster unit. There did not appear to be any anomalous readings on the pad surface of high conductivity at the time of inspection. There were some higher readings but all were in containment. A bucket had readings of 12 mS. Well cellars did not have any high conductivity readings. Some examples were .48mS, .91mS, .89mS, .99mS. Readings in production tank containment were 1.9 |

During operations on site the pad had quite a considerable amount of mud on it but that was not in evidence during the inspection for NOT.

Not sure how long it was leaking. There are no reports of prior leaking. We used absorbent pads to mop up the spill. The well was shut in, and the thread on the hammer union was sealed.
