There were 5 active wells on the pad; the 1H, 5H, and 6H all had production trees in place with pressure on them but were shut in to the production line out. The 1H was showing a pressure of 1423psi at crown gauge and 1429 at the wing valve. The 5H showed 1829 psi at the wing valve gauge. Production line valve was shut with -5 psi on gauge on production line. The 6H was also shut in from production with a pressure of 1774psi on gauge on wing valve. The cellars on these wells were completely filled with gravel. The 2H and 4H were not in production at time of inspection. Entire BOP stack was present on these wells. Workover completions foreman said they were waiting on rig to come in and run tubing on these wells. Cellars were filled with water and free of bubbling. |

Present during the inspection were myself, Jon Sassi, Department EPS, and Eric Rooney, Department Professional Geologist. The inspection revealed that the pad was being utilized as a remote pump station for hydraulic fracturing activities for other locations. As evidenced in the attached photograph, the containment for the pump appeared to be adequate. Conductivity readings conducted around the pump station suggested that the spill was indeed a fresh water spill. No further action will be taken by the Department. |

The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 467psi on the production casing and 399psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:35p.m. |
