This inspection is related to a nearby private water supply complaint, complaint ID 302271. Department records indicate that only one well has been drilled and stimulated at this time, the Bienko 4H (115-20969). Other wells that have been permitted for this pad are: 1H (115-21414), 2H (115-21415), 3H (115-21416), 5H (115-21417), and 6H (115-21418).
2353550 Primary Facility 2015-03-17 Routine/Complete Inspection No Violations Noted I visited the Falcon 27 drilling rig on March 17 2015 @ 3pm. Rig was in process of drilling the intermediate section of hole on the 2H at time of inspection. Other wells on the Bienko site include the 3H, 5H, 6H, and 8H, however access to these wells were limited due to active drilling activities on site. Only conductors have been set for these wells. Will proceed with further inspection as rig skids to these holes.

The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water and ice above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge and a 1000psi pressure gauge with 0psi on it, the valve going to the production tank was closed. There was 671psi on the production casing and 374psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:05p.m.

2669086 Primary Facility 2017-12-04 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. There was water above the (20x13) and (13x9) annuli and there was no bubbling noticed. I noticed a sheen on top of the water and contacted Steve Moyer of SWN to have it vacced out. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2767811 Primary Facility 2018-08-27 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water .5' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
