Well permits that have been issued for this pad are the Salt Lick Hunting Club 1H (115-21423), Salt Lick Hunting Club 2H (115-21424), and the Salt Lick Hunting Club 3H (115-21425). All perimeter BMP’s in place to include red lined additions. Pad is 100% complete. No violations noted.

2262530 Primary Facility 2014-02-27 Incident- Response to Accident or Event No Violations Noted On 2/27/14, SWN contacted the Department and reported that approximately 1 bbl of oil based mud had been spilled on containment at their RU 59 Salt Lick Hunting Club Pad. Arrived on site at approximately 1500 hours. Met with the company man who explained that at about 4 AM, they had identified that a valve related to their shaker unit had frozen. They proceeded to unfreeze the valve and open it. While changing out a container, drill mud backed up in the shaker line and spilled onto the containment. They then identified that the valve was still frozen shut when they thought it was open. They then removed the released drill mud and power washed the affected area. An inspection of the containment at the shaker location by the Department revealed that the containment does appear to be sound. No further action by SWN is required at this time. The rig on-site is the SWN Rig # 10. SWN is currently drilling the 3H and is not presently scheduled to drill any other wells on this pad at this time.
2352035 Primary Facility 2015-03-10 Routine/Complete Inspection No Violations Noted I visited the Salt Lick Hunting pad with the 1H, 2H, and 3H wells on it on March 10th 2015 at about 11am. There were three wells on the pad, the 3H being a producing well. Production tree on the 1H was in place with 812 psi on the production line out with valve in the open position. A pressure of 1114 was indicated on gauge at wing valve. The cellar was tarped and covered with snow. The 1H and 2H only had 20” conductors set to 40’.

The permit number on the well was wrong, I contacted Afton Sterling of SWN and she informed me it would be corrected. I took a walk around the location and everything looked OK.

I left the site at 10:45a.m.

2585069 Primary Facility 2017-04-20 Routine/Complete Inspection No Violations Noted Todays inspection, 4/4/17 was conducted as a routine inspection. Site is stable. Greater than 70% vegetation has been established. All three wells appear to be in production. Well pad is fenced in and locked. No violations noted.

The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 541psi on the production casing and 505psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:15p.m.

The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access with water .5' below grade and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
