2564539 Primary Facility 2017-02-22 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 14:50p.m.

The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure and the valve going to the production tank was closed. There was 482psi on the production casing and 344psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 15:20p.m.

2659069 Primary Facility 2017-11-09 Routine/Complete Inspection No Violations Noted At time of inspection.
