2403972 Primary Facility 2015-08-27 Preoperation Inspection No Violations Noted On 8/27/15, the Department was on site at the SWN NR 14 Brant Well Pad to conduct a Preoperational Inspection. Inspection conducted with Jon Sassi, Department EPS, Rich Gulyas SWN Construction Foreman, and Andy Breese, SWN Construction Foreman. The well permits that have been issued for this pad are the Brant N 4H (115-21648) and the Brant S 5H (115-21649). All perimeter BMP’s in place to include red lined additions. Pad is 100% complete. The pipeline company tying in the pad is utilizing the pad to stage equipment. A section of an infiltration berm has been washed out and needs to be repaired.
2533937 Primary Facility 2016-10-31 Incident- Response to Accident or Event No Violations Noted Michael Williams, Environmental Health and Safety Specialist of SWN contacted PADEP NERO Emergency Response Team in Wilkes-Barre, PA at 10:30 PM on 10/30/16 to report a spill of (3) barrels, (125) gallons of hydraulic fracturing fluid. Bob Bisignani, Assistant Emergency Response Coordinator in NERO contacted the PADEP Scranton District Office at 8:45 on 10/31/16 to report the spill to the Oil and Gas Management Program. The complaint details in the complaint inspection form state the following. "NR4 Brandt Pad lost 125 gallons onto containment of frac water (sand laden liquid) and when cleaning up, a ½ inch hole was discovered in containment. Contractor was called in to clean up. Spill occurred Oct. 30 at 10:30 PM." Mike Deming Senior Regulatory Analyst of SWN contacted me at 9 AM on 10/31/16 regarding the spill and to confirm that the Oil and Gas Program in the Scranton District Office was notified of the release. Mr. Deming stated the following information in reference to the spill during our phone con
2555033 Primary Facility 2017-01-25 Administrative/File Review No Violations Noted On 1/19/17 at 16:06, Mike Deming, Senior Regulatory Analyst of Southwestern Energy Production Company submitted a report to the Department in reference to an investigation conducted for a potential release of hydraulic fracturing fluid to the Brant (North Range Unit 14) pad surface on 10/30/16. The Department has completed a review of the field notes, lab report and tabulated results of the investigation conducted by SWNPC and has concluded that no further action is warranted of SWNPC at this time.
