Proper site identification was present at the edge of the lease road as well as permit numbers for the Phelps 1H and 4H wells. On site, the 1H had portion of stack set on casing with the well tag permanently fixed. The rest of the cellar on the 1H was covered in snow and the 4H was completely covered in snow including the bop. Both wells had cattleguard in place. There were no visible gauges. |

The weather conditions were partly cloudy skies and breezy with temperatures in the mid to upper 40's. There was no activity at the site during the inspection. There were (2) representatives from Woodard and Curran, a subcontractor of Chief, at the site either conducting an inspection or in the process of surveying. On 10/20/17, Peg Farrell, Environmental Manager of Chief contacted me stating that hydraulic fracturing (stimulation) will not take place in the near future as initially planned and there are frac tanks on this pad containing reuse water intended be used during the frac job that would need to be moved to another pad. On 10/30/17 she forwarded an email to me as per my request which stated the following. "As mentioned recently by phone, there is reuse water currently staged in 28 frac tanks on the Phelps B because we were going to frac there, but the schedule has changed and we will not be fracing there in the foreseeable future. The proposed modifications were approved by the Department on June 5, 2018. Photo's #2 and #3 show Infiltration Basin 1,a conversion of Sediment Trap 1. This basin appears to have been built to specs. Photo #6 shows crews working to convert Sediment Trap 2 into Infiltration Basin 2. No E&S issues were identified. No violations noted. |
