The area was partially snow covered and there were no erosional issues apparent at this time, and the site was stable. The most recent E&S Inspection was on 3/26/15. 2466941 Primary Facility 2016-03-22 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 10:45 a.m., the cellar was about half full of water with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was about 660psi on the production casing. I took a walk around the location and everything looked OK.

I left the site at 11:30 a.m.

2475035 Primary Facility 2016-03-21 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a routine complete inspection of the Grasavage E. Well site (ESX13-115-0105). There were no erosional issues apparent at this time, and the site was stable.

2526031 Primary Facility 2016-09-22 Routine/Partial Inspection No Violations Noted
2545883 Primary Facility 2016-12-16 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:10a.m., the cellar had ice and snow at the bottom. The (13x9) vent pipe was open and venting into a small plastic tank, the (9x5) vent pipe was open and venting into a small plastic tank. There was 624psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 11:45a.m.

2556290 Primary Facility 2017-01-25 Follow-up Inspection No Violations Noted The Department was on site at the Grasavage, E. Well Site located at 2993 East Rush Street Jessup Township, Susquehanna County for a Follow-Up Inspection to the Department’s 9/22/2016 inspection. I reviewed the E&S Inspection Log and found that Mike Brilla, TEC conducted the most recent inspection, which was noted as a post precipitation inspection, on 1/25/2016.

During this inspection, the Department inspected the sediment trap that was found to not have sufficient stabilization during the previous inspection. At this time, the sediment trap was filled with water and ice but erosion control blanket was visible through some areas.

The well site was about 90% snow covered during the inspection. No obvious issues were found.

2634883 Primary Facility 2017-09-11 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. All the annuli vent pipes were venting into the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2714128 Primary Facility 2018-04-03 Site Restoration No Violations Noted I inspected the Grasavage E Well Site .The cut and fill slopes were stable and had adequate vegetation in excess of 70% perennial cover. There were no erosional issues observed. The wells were properly labeled.

2728777 Primary Facility 2018-05-11 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Grasavage well site. The wells associated with this site are as follows: 115-21712, 115-21713, 115-21714, 115-21716.

These wells were properly labeled.

The cut slope was well vegetated. It was stable. The fill slope was stable and very well vegetated. There were no erosional issues noted.

The pad surface and access road were stable and solid.

There were no violations observed.

2743652 Primary Facility 2018-06-20 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 5.5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank at 24% LEL and 4% methane. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2819785 Primary Facility 2018-12-21 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Grasavage well site. The weather at the time of inspection was above 60 degrees F. The sky was partly sunny and it had rained earlier in the morning. The well pad surface was damp but otherwise solid.
    The access road and pad surface were both solid and stable.  The pad surface was recently made smaller and the vegetation around the edge was doing well, as it was lush and thick.
    The cut and fill slopes were in good shape with no rills or erosional issues.  The slopes were well vegetated and stable. 
    The wells were clearly and properly labeled. 
    The infiltration basins were well vegetated and appeared to be performing their jobs.
    There were no violations noted.
2839650 Primary Facility 2019-02-07 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water and ice 6' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 2/5/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
