2491955 | Primary Facility | 2016-06-02 | Routine/Complete Inspection | No Violations Noted | I conducted an inspection of the site accompanied by Scott Kuchta, DEP O&G WQS. Flowback operations were in progress as we arrived at 12:15 PM. Oil States Energy Services is the Chief subcontractor conducting the flowback. Shortly after our arrival, we met with Mason Hope, well tester, of Oil States. Mr. Hope stated the following information. "Flowback of the Clark unit 1H well started on 5/30/16. The well is making water and gas, with the gas being sent directly to sales. Thus far 3,000 barrels of fluid have been recovered at a rate of 40 barrels/hour. The fluid is sent to frac tanks on-site in a tank battery. Water from the sand trap is sent to the gas buster tank. A line heater and separate is used in the flowback process. The frac tanks were -site before the flowback started. The 3H well is in production. The 2H well has not been stimulated and is shut in."
I recorded 3.7 milliSiemens/cm³ conductivity in water in the frac tank battery containment in the Northeast section of the pad. There were |

2564071 | Primary Facility | 2017-02-21 | Incident- Response to Accident or Event | Viol(s) Noted & Immediately Corrected | The operator reported a release of brine water to the ground from the Clark 3H Well on 21FEB2016 at 10:36 AM. The reported release was estimated to be 3 gallons maximum. The brine was reported to have been released as a mist from the Gas Buster Tank while conducting coiled tubing operations.
The operator further detailed remediation of the release consisted of establishing an area of 50' x 50' where the greatest potential for contamination to have occurred. The area was screened by observation and TDS screening. The estimated amount of the release been reported as a conservative estimated of no more than three gallons. The potentially contaminated material was excavated and disposed of by Waste Management. The operator would provide a final report of the incident. I conducted the site visit and had met with Aaron Stredney, Environmental Inspector for Chief Oil & Gas. The reporting and response to the spill by the operator appears to have diminished the potential of this incident to cause environmen |
