Inspection conducted with Rich Gulyas, SWN Construction Foreman. The well permit that has been issued for this pad is the Colwell 3H (115-21788). All perimeter BMP’s in place to include red lined additions. Pad is 95% complete. No violations noted.
2315033 Primary Facility 2014-09-10 Routine/Complete Inspection No Violations Noted On 9/10/14, the Department was on-site to evaluate clean-up activities of a small spill that was reported by SWN on 9/9/14. The spill was estimated to be between 5 to 8 gallons of hydraulic fluid that had been lost when a line ruptured on a D6 bulldozer. The area has been excavated with the contaminated material having been placed in roll off containers. The excavation was screened with a PID. Soil samples were collected by SWN to determine efficacy of the current remediation effort.
2315715 Primary Facility 2014-10-27 Routine/Complete Inspection No Violations Noted On 9/8/14, SWN reported an approximately 7 gallon release of hydraulic fluid to the ground from an excavation machine due to a mechanical failure. SWN emailed an electronic copy of their Investigation Summary Report. Attached is a copy of the report. After a review of the report, the Department concludes that the spill response was adequate and that no further action is required by the Department and SWN. There was 505psi on the production casing and 311psi on the production line. I took a walk around the location and there were no obvious concerns noticed.
