Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2307840 Primary Facility 2014-09-26 Preoperation Inspection No Violations Noted The Department was on site at the SWN NR03 Colwell West Pad to conduct a Preoperational Inspection. Inspection conducted with Rich Gulyas, SWN Construction Foreman. The well permit that has been issued for this pad is the Colwell West 2H (115-21829). All perimeter BMP’s in place to include red lined additions. Pad is 100% complete. No violations noted.
2316003 Primary Facility 2014-10-08 Drilling/Alteration No Violations Noted Condcutor set.
2371430 Primary Facility 2015-05-19 Routine/Complete Inspection No Violations Noted
2438446 Primary Facility 2016-01-06 Routine/Complete Inspection No Violations Noted

2490699 Primary Facility 2016-06-06 Routine/Complete Inspection No Violations Noted On 6 June 2016, the Department conducted an inspection of the SWN NR 03 Colwell West Well Pad. Only one well, the 2H, 115-21829 has been drilled and is in production. The pad is both stone and vegetative stabilized. No violations noted.
2493928 Primary Facility 2016-06-17 Routine/Complete Inspection No Violations Noted
2502518 Primary Facility 2016-07-13 Routine/Complete Inspection No Violations Noted On 13 July 2016, the Department conducted an inspection of the SWN NR 03 Colwell West Pad to evaluate if the pad meets the criteria necessary to terminate its ESCGP. Also present during the inspection were Mike Deming, Rich Gulyas, and Jason Petlock, SWN representatives. A review of the pad stability revealed that permanent stabilization has been attained. A review of the permit revealed that the site fairly matches the permit site restoration plan. Hence, if a technically complete NOT application was submitted for this pad, the application would probably be approved.
2600912 Primary Facility 2017-06-02 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 8:50a.m.

The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 912psi on the production casing and 732psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 9:25a.m.

2685315 Primary Facility 2018-01-18 Routine/Complete Inspection No Violations Noted Inspection conducted as a routine inspection. Site snow covered. Site is plowed. Well tender reviewed well heads during the inspection. No E&S violations observed. No spills detected. No violations noted.
2709562 Primary Facility 2018-03-19 Routine/Complete Inspection No Violations Noted At time of inspection.

There was a crew on location and taking the cage off from around the well with a crane. The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (13x9) annuli were below the water level and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2807709 Primary Facility 2018-11-28 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water .5' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2836131 Primary Facility 2019-02-05 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. Overall the site has good stabilization. No E&S issues identified. No violations noted.
