2423742 | Primary Facility | 2015-10-07 | Routine/Complete Inspection | No Violations Noted | On 10/7/15, the Department conducted an inspection of the SWN Nowicki Pad. To enter the access road, one has to drive through the Endless Mountains Stone Company Yard. Crews are on-site installing production equipment. Ditches are open with piping laid out to both well heads. 10 frac tanks on-site. 70% of the pad liner still in place. SWN did report a loss of about 2 barrels of flowback water to containment the week prior to this inspection. Containment that is still in place looks sound. Pad surface where containment has been removed shown no sign of contamination. No violations noted. |

2570649 | Primary Facility | 2017-03-09 | Routine/Complete Inspection | No Violations Noted | At time of inspection, arrived on site at 13:45p.m.
The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge and a 1000psi pressure gauge with about 10psi on it, the valve going to the production tank was closed. There was 856psi on the production casing and 398psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:15p.m. |
2616969 | Primary Facility | 2017-06-28 | Routine/Complete Inspection | No Violations Noted | The Department was on site to conduct a Routine Complete Inspection. I reviewed the on-site E&S Inspection Log and found that the most recent inspection was conducted by SRT on 6/21/17. |
2665882 | Primary Facility | 2017-11-29 | Routine/Complete Inspection | No Violations Noted | At time of inspection.
The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (9x5) annuli were below the water level and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted. |
2765621 | Primary Facility | 2018-08-21 | Routine/Complete Inspection | No Violations Noted | At time of inspection.
The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water .5' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted. |
2816450 | Primary Facility | 2018-12-17 | Routine/Complete Inspection | No Violations Noted | Today's inspection was conducted as a routine inspection. No E&S issues identified. Good vegetative stabilization present. No violations present. |
2850978 | Primary Facility | 2019-02-19 | Incident- Response to Accident or Event | No Violations Noted | I arrived onsite at 1400 hours to conduct an incident response inspection after receiving a spill report stating that 30 gallons of fresh water mixed with fine sediments from drill cuttings had been inadvertently released from a water truck and impacted a portion of the pad's access road. The weather was sunny and cold at 25 degrees F with calm winds and no precipitation. Drilling operations were underway utilizing SWN's Rig 45.
Upon my arrival, I entered the company man trailer to check in. No one was inside, so I asked several nearby workers where I could find him. One of them stated that he believed the company man was on the rig floor. Since I did not want to go up there unaccompanied, I asked the workers to radio him for me. They were not equipped with radios, but one of them offered to walk up to the rig floor and inform him I was there for an inspection. After several minutes, the company man eventually walked down and introduced himself as Ben Dodd. As Mr. Dodd and I entered his trailer, I informed |
