2333551 Primary Facility 2014-12-16 Preoperation Inspection No Violations Noted On 12/16/14, the Department was on site at the SWN NR 16 Haley Well Pad to conduct a Preoperational Inspection. Inspection conducted with Rich Gulyas, SWN Construction Foreman and Ryan Klemish, Regulatory Analyst. The well permits that have been issued for this pad are the Haley North 2H (115-21863) and the Haley 4H (115-21864). All perimeter BMP’s in place to include red lined additions. Pad is 95% complete. The cross culvert located near the entrance to the pad has not been installed. Mr. Gulyas asked if the cross culvert could be eliminated since field conditions suggest no flow will ever go through the culvert. The Department agrees with that assessment and will not require that the culvert be installed. No violations noted.
2354539 Primary Facility 2015-03-24 Routine/Complete Inspection No Violations Noted I inspected the Haley Pad on 03/24/2015 at 10:00 am. Two permits posted at entry for the North 2H and South 4H. According to drillers log in mailbox, the 2H had a 40’ conductor set, a 655’ surface string cemented with 340 sks or cement pumped at 15.8 ppg with 30 bbls circulated to surface. There was 1619’ of intermediate casing cemented with 310 sks of cement at 15.8 ppg with 15 bbls to surface. Rig encountered water at 405’ at 10 bbls/hr. Bop tested good for 3000 psi, FIT for 810psi or 18#EMW. The 4H well had a 40’ conductor, 656’ 133/8 casing cemented with 335 sks with 15 bbls returned. Water encountered at 484’ at 10 bbls/hr. BOP was tested at 3000 psi on 1/31/2015. FIT @ 808psi. Hole was loaded with 12ppg obm. Both wells had bottom section of stack set on 95/8 casing. The 2H was tagged as drilled by Minichi 9. Cellars were filled with snow, cattleguards around cellars.

The cellar was set up with a plastic corrugated pipe around the well for inspection access and was about a quarter full of ice. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge and a Schneider digital pressure gauge with 196psi on it, the valve going to the production tank was closed. There was 463psi on the production casing and 305psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:05p.m.

The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (13 x9) annuli were below the water level and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water 1.5' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2814268 Primary Facility 2018-12-12 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. Access road in good condition. Very good vegetative stabilization of pad slopes. No E&S issues noted. No violations noted.
