They are monitoring pressure on the backside in the frack van, approx. 950 psi today. Plan going forward is to close sleeve, pull tubing, run caliper log on 5 1/2" casing and remediate accordingly

2545732 Primary Facility 2016-12-19 Site Restoration No Violations Noted The Department was on site at the NR#24 Buckhorn well site in response to the submittal of a Notice of Termination to conduct a Restoration Inspection. The E&S Inspection Checklist Site Inspection Log indicated that Seth Tratthen conducted the most recent inspection on 12/7/2016 and that no attention was needed. Rich Gulyas and Seth Tratthen (SWN) were present for the inspection.

The site appears the meet permanent stabilization requirements. There are two timber stockpiles on either side of the access road that require a landowner signoff if they are to remain in place. Upon receipt of the landowner signoff, I recommend that this NOT be approved.

2549635 Primary Facility 2017-01-04 Follow-up Inspection No Violations Noted At time of inspection, arrived on site at 10:00a.m. for a follow up inspection to the Incident Response inspection conducted on 1/13/16. I met with Brad Noble of SWN. The cellar was set up with a plastic corrugated pipe around the well with gravel between the pipe and the perimeter of the cellar. There was water above the surface casing with no bubbling noticed. The (9x5) vent pipe was open and set up with a Taylor plug, the valve going to the production tank was closed, Brad used a Crystal pressure gauge to check the pressure and he got a reading of minus 9.3psi. There was 26.5psi on the production casing and 416psi on the production line. Brad informed me that a packer was put on the tubing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 11:10a.m.

2774425 Primary Facility 2018-09-06 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a small spill complaint inspection after Mike Deming, SWN Senior Regulatory Analyst contacted the Department on 9/5/18 at approximately 3:05 PM to report that on September 5, SWN Regulatory was notified by SWN Production of an apparent dark stain with associated white crystallization on the Buckhorn pad surface after the ground near the wellhead, where secondary containment was removed, began to dry in the sun. After further visual observations were made by regulatory, it was determined the release was likely greater than five (5) gallons off containment and the Department was immediately notified. It was reported as an area approximately 30’ feet x 30’ feet, and likely caused by a small unnoticed compromise in secondary containment in or near the stained area that allowed some fluids to escape during completions operations.

SWN contracted Woodard & Curran, an environmental consultant to oversee the remediation of this spill. A technician from Woodard and Curran confirm

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water 2.5' below grade in the cellar and there was no bubbling noticed. There were 23 empty 500bbl tanks on containment from a recent frac of the 2H, 3H and 5H wells, there was snow and ice on the containment making inspection of the containment limited. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
