2380094 Primary Facility 2015-04-29 Preoperation Inspection No Violations Noted On 4/29/15, the Department was on site at the SWN NR 18 Oak Ridge Well Pad to conduct a Preoperational Inspection. Inspection conducted with Rich Gulyas, SWN Construction Foreman. The well permits that have been issued for this pad are the Oak Ridge North 2H(115-21897), and the NE 3H(115-21675). All perimeter BMP’s in place to include red lined additions. Pad is 95% complete. No violations noted.
2380497 Primary Facility 2015-06-11 Routine/Complete Inspection No Violations Noted Falcon Drilling Rig #25 is currently drilling the Oak Ridge 2H well @ (1,647) feet. It appears as if cement overtopped the cement tank and spilled onto containment and compromised liner integrity is noticed in the containment near (2) diesel fuel tanks. The containment in this particular area consists of a layer of felt above a (40) mil HDPE Western liner, and a layer of felt below the liner. The pad surface is revealed below the tears in the felt and the liner. Mr. Fooshee stated that he would have the area cleaned up where the cement was spilled to containment and the tears in the containment will be repaired.

2402876 Primary Facility 2015-08-27 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected Mike Williams of Southwestern Energy contacted the PADEP Northeast Regional Office Emergency Response Team at 9:29 PM on 8/25/15 to report an estimated (20) gallon diesel fuel release that occurred and impacted the access road on 8/25/15 around 8:45 PM. Hydraulic fracturing operations were in progress on the Oak Ridge Pad when the release occurred. The release occurred due to a sand track shearing the fuel line connecting the fuel tanks on the truck as the truck was travelling up the access road approaching the pad. In order to stop the leaking diesel fuel from the truck and control the release, the remaining diesel fuel in the truck was unloaded into nearby water trucks, drip pans were placed under the truck and absorbent pads were utilized to soak up the diesel fuel on the ground surface.

2406290 Primary Facility 2015-09-11 Incident- Response to Accident or Event No Violations Noted On 8/24/15, the Department conducted an inspection of the SWN NR 18 Oak Ridge Well Pad in response to their self-reporting of a small spill of fresh water to containment. Present during the inspection was Ryan Klemish, SWN Regulatory Analyst. The spill occurred during stimulation activities on the pad. The spill occurred when a valve was not fully closed and caused a frac tank to overflow. A review of the containment revealed that the containment looked sound with no tears or punctures present. The water was immediately vacuumed up. No further action is required of SWN at this time.

At the time of the inspection, it was noted that SRT signed off on an E&S inspection on 6/6/16. The Department walked the well site to survey the temporary stabilization. Some of the vegetation on the cut slope appears to be dead. This area should be monitored to ensure that potential erosion issues do not occur once the dead vegetation decays. Also, a small area near the tree line appeared to be recently mulched.

2563262 Primary Facility 2017-02-21 Routine/Complete Inspection No Violations Noted Last inspection 2/16/2017. The NR18 1H is not in production but the 2H is. The access road to this site is long but in good shape, despite recent snowmelt and warm weather. The cut and fill slope are vegetated and stable and the pad surface is graded well. The area has some snowcover but vegetation is visible. There are no apparent erosion issues at this time.

2575181 Primary Facility 2017-03-24 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 9:30a.m.

The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water and ice above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 606psi on the production casing and 307psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 10:05a.m.

The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (13x9) annuli were below the water level and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water 3' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
