2535825 | Primary Facility | 2016-11-10 | Routine/Complete Inspection | No Violations Noted | At time of inspection, arrived on site at 9:10a.m., Baker Hughes was on site and fracking the 1H, 2H, and 6H. I met with company man Wayne Wilridge (Operations Manager). They were fracking stage 19 out of 28 on the 2H and running a plug and perforation in the 6H. The 1H was completed with 21 stages and 27 stages were completed out of 32 on the 6H. The water was being hauled to tanks on the lower pad and pumped to the well pad through a plastic line. The tanks were on containment and it was in good condition. I took a walk around the location and there were no obvious concerns noticed.
I left the site at 10:40a.m. |
2552783 | Primary Facility | 2017-01-17 | Routine/Complete Inspection | No Violations Noted | At time of inspection, arrived on site at 11:20a.m., the well was recently completed and is in production. The well tag was on the well and correct. The cellar was full of water with no bubbling noticed, the (20x13) vent pipe was open and venting into a small plastic tank, the (13x9) vent pipe was open and venting into a small plastic tank, the (9x5) vent pipe was open and venting into a small plastic tank. There was 762psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.
I left the site at 11:55a.m. |
2557872 | Primary Facility | 2017-01-26 | Routine/Complete Inspection | No Violations Noted | The Department was on site to conduct a Routine Complete Inspection of the Wright well site (ESX13-115-0131). The wells associated with this site are as follows:
115-21915 115-21916 115-21919 115-21920 The Department had previously been on site on January 18, 2017 and there was debris on site at that time. There were sandbags littering the site and there was very little growth on the fill slope of the tank pad (Photo 1). There apparently had been recent excavation work done at that time near the road entrance, and there was an expanse of unmulched/unstabilized ground (Photo 2). At the time, the Department decided to wait and see if the situation was just in limbo due to work having been completed for the day or weather conditions. On January 26, 2016, the Department returned to the site and found the previously noted conditions were the same. The exposed ground was not stabilized nor was any debris removed from the pad site. The fill slope was also not stabilized. It would be recommended that the fi |
2600178 | Primary Facility | 2017-05-16 | Routine/Complete Inspection | No Violations Noted | The Department was on site to conduct a Routine Complete Inspection. Upon my arrival I reviewed the E&S Inspection Log to find that the most recent inspection has been conducted on 5/15/17 by Sam Wiernuscz and it was a post precipitation inspection.
During the course of the Department's inspection, I conducted a visual inspection of the wells. I found bubbles forming at the unions on the bull plug, with a gauge, on the production tree of the 2H and 6H wells. |
2627479 | Primary Facility | 2017-08-21 | Follow-up Inspection | No Violations Noted | I received an email from Mike Pirner of Cabot letting me know most repairs to the Wright well site were now completed. Upon inspection of the site I found that they were indeed completed and stability had been restored to the site previously affected by torrential rains. The entire area was now reseeded, limed, fertilized and mulched. The slopes were starting to green up where erosion blanket had been placed and the sediment basins were reshaped, reseeded and mulched.
There were no violations noted. |
2641091 | Primary Facility | 2017-09-28 | Routine/Complete Inspection | No Violations Noted | At time of inspection.
The well was in production. All the annulus vent pipes were connected together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted. |
2651594 | Primary Facility | 2017-10-06 | Routine/Complete Inspection | No Violations Noted | Scott Rockwell (TEC) conducted the most recent inspection on 10/6/2017. |
2705529 | Primary Facility | 2018-03-13 | Routine/Complete Inspection | No Violations Noted | I inspected the Wright well pad on March 13, 2017. The weather was snowy and damp. The access road was muddy but there was a sweeper unit on hand. The cut and fill slopes were stable and the pad surface was muddy but stable. There were some Keane trucks staged on site.
The wells associated with this site are as follows: 115-21915 115-21916 115-21920 are all active 115-21919 plugged. The wells were all properly labeled. There were no erosional issues apparent. There were no violations |
2747106 | Primary Facility | 2018-06-29 | Routine/Complete Inspection | No Violations Noted | At time of inspection.
The well was in production. There was water 6' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was no gas detected at the poly vent tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted. |
2804125 | Primary Facility | 2018-11-15 | Site Restoration | No Violations Noted | I performed a RTNC inspection of the Wright well pad in the course of performing a Notice of Termination inspection for the Wright well connect project.
The weather was cloudy and the temperature was about 27 degrees F. Snow was imminent. The access road was stable and solid as was the pad surface (Photo 1). There were no erosional issues of note on these areas. Active agriculture was observed in the vicinity of the wellpad, with no adverse impacts noted. The slopes and area surrounding the wellpad was well vegetated and the vegetation was healthy and abundant. There were no areas of instability noted (Photos 2 and 3). Previously during very wet conditions there had been some erosional issues at the site but the areas affected have long since been corrected and were all growing well and stable. There were no adverse erosional issues noted at the time of inspection. The wells were all correctly labeled (Photo 4). There were no violations observed. |

The well was in production. There was ice 3' below grade in the cellar and there was no gas detected in the remainder of the cellar. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was no gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 2/5/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted. |
